Your Opportunity:The Ambulatory Pharmacist manages medication therapy for outpatients within Alberta Health Services facilities in which he/she, both initially and on an ongoing basis, performs a comprehensive medication review to identify, resolve and prevent medication-related problems. This is accomplished through collaborating with members of the health care team including the patient and/or their caregivers to formulate a medication treatment plan and work to optimize patient outcomes. Duties will include selecting, initiating, modifying, or administering medication therapy, monitoring the patient\'s response to therapy, and documenting all direct patient care activities. Provision of verbal education to patients as well as coordination and integration of medication therapy management services is also an expectation. The Ambulatory Pharmacist is responsible for the provision of pharmaceutical care according to AHS, hospital and departmental standards. In addition to direct patient care, the Clinical Pharmacist is also responsible for drug information, education and research activities. They are responsible for maintaining their own professional development and participating in the precepting and mentoring of other staff and students. As well, the Ambulatory Pharmacist will assist with the planning, delivery and implementation of ongoing quality/healthcare improvement initiatives and research programs.Description:As a Pharmacist I, you will be responsible for the safe, effective and efficient preparation and distribution of pharmaceutical services.
