ACH was built with family-centred care in mind and prides itself on engaging patients and families regularly through a family-centred approach to care. ACH is a leader in pediatric and neonatal care and is a major referral center for pediatric malignancies and complex hematological treatment including pediatric bone marrow transplants. Alberta's only hospice for children (Rotary Flames House) is located next door to ACH. The Pharmacist is responsible for direct patient care services, drug distribution services (indirect patient care), drug information & education/research activities. Also includes precepting & teaching pharmacy residents, staff, and students and provision of drug information to the pharmacy, nursing, medicine & other members of the multidisciplinary team as required. Pharmacists actively participate in patient care rounds, liaise with members of the health care team & develop professional relationships with patients & families to formulate a medication treatment plan. Direct patient care responsibility could be in general pediatrics, and HOT (hematology, oncology, transplant). Mental Health, PIC, NICU or Emergency. 2-5 yrs experience is recommended: Within hospital pharmacy. With pharmacy computer order entry system. Unit dose drug distribution. Computer medication order entry, IV admixture & unit dose drug distribution experience & Windows-based computer applications experience is recommended. You must provide a letter of intent and indicate the reason you are applying to ACH Pharmacy Services. If selected for an interview, be prepared to provide a Pharmaceutical Care Plan that you have recently completed.

Description:

As a Pharmacist I, you will be responsible for the safe, effective and efficient preparation and distribution of pharmaceutical services. You will manage medication therapy for patients, both initially and on an ongoing basis. In addition, you will provide assessment, intervention and resolution of potential or actual drug-related problems and oversee activities related to compounding and packaging.

Required Qualifications:

Bachelor of Science in Pharmacy. Active or eligible for registration with the Alberta College of Pharmacy (ACP). Additional Required Qualifications:

Advanced Prescribing Authority (APA) is required (for new practitioners; must obtain within one (1) year). Recent direct patient care experience preferred (at least 1 year within the last 3 years). Fluency in the English language (both written and verbal). Demonstrates computer literacy for drug information and word processing. Demonstrates ability to use evidence-based medicine in direct patient care. Demonstrates self-directed learning and continuing education. Capable of making decisions and working independently. Demonstrates good time management including the ability to switch priorities and multitask. Demonstrates excellent communication and interpersonal skills, and good organizational and problem-solving skills. Self-motivated; able to work both independently and cooperatively in a team environment. Working to the full scope of practice including but not limited to, APA, laboratory ordering and authorization to administer injections. Additional Preferred Qualifications: Experience with computer medication order entry, IV admixture and unit dose drug distribution is an asset. Experience with Windows-based computer applications is an asset. Project experience and committee work an assets. Member of a voluntary professional organization is an asset. Preferred Qualifications:

2+ years of recent experience within hospital pharmacy preferred. 2+ years experience with pharmacy computer order entry system preferred. 2+ years experience with unit dose drug distribution preferred. Extensive clinical experience preferred. Recent experience as a member of a multidisciplinary health care team preferred. Hospital pharmacy residence and/or prior hospital experience preferred Experience in pediatric/oncology/neonatal drug therapy preferred.
