Your Opportunity:

The pharmacist manages medication therapy for AHS patients in which he/she, both initially and on an ongoing basis, assesses patients for potential or actual drug-related problems and, working within a collaborative team environment, intervenes to resolve or prevent drug-related problems in order to optimize patient outcomes. This is accomplished through actively participating in patient care rounds, liaising with members of the health care team, and developing a professional relationship with the patient and their family to formulate a medication treatment plan. This also entails implementing that plan including selecting, initiating, modifying, prescribing, adapting, or administering medication therapy, documenting expected and real outcomes and documenting clinical interventions. In addition, the pharmacist is responsible for maintaining their own professional development and participating in the precepting and mentoring of other staff and students, ensuring accurate and efficient dispensing of medication including providing functional supervision for pharmacy operations, and performing administrative responsibilities. Recent direct patient care experience as well as experience with the provision of comprehensive pharmaceutical care within the hospital setting is ideal.

Required Qualifications:

Bachelor of Science in Pharmacy. Active or eligible for registration with the Alberta College of Pharmacy (ACP). Additional Required Qualifications:

Bachelor of Science in Pharmacy from a recognized institution required. Licensed or eligibility for licensure with Alberta College of Pharmacists, Certificate of competency from the Pharmacy Examining Board of Canada required. Additional Prescribing Authorization granted by the Alberta College of Pharmacists required. Active membership in the Canadian Society of Hospital Pharmacists is recommended. Certification to administer drugs by Injection granted by the Alberta College of Pharmacists required. Demonstrates ability to use evidence-based medicine in direct patient care. Demonstrates computer literacy for drug information and word processing. Capable of making decisions, working independently or with a team and good time management, Excellent communication, interpersonal skills, organization and problem-solving skills. Fluency in the English language (written and verbal). Accountability. Preferred Qualifications:

Active membership in the Canadian Society of Hospital Pharmacists. Advanced Clinical Pharmacy Degree. Experience with: Comprehensive pharmaceutical care in a hospital setting. Computer medication order entry, IV admixture & unit dose drug distribution. Hospital pharmacy residency & prior hospital experience. Accredited Canadian Pharmacy Residency certificate. Recent exp. as a member of a multidisciplinary patient health care team. 2 - 4 years clinical exp. within a specialized program area.
