Job Description

The Emergency Department Clinical Pharmacist is a member of the multidisciplinary healthcare team providing pharmaceutical care to both inpatients and outpatients in a fast-paced environment, including collaborative practice with specialized programs. The Pharmacist works collaboratively with the patient, family and health care team to optimize medication use, avoid medication-related problems and achieve desired patient outcomes. The Pharmacist provides drug information, promotes evidence-based/cost effective drug therapy, promotes clinically relevant research and is involved in education for students, families and staff. Ongoing collaboration and communication within the healthcare team and with the pharmacy team is essential. The Pharmacist is an effective manager of time and resources, supervising and delegating activities to pharmacy technicians to support patient care where appropriate.

Qualifications

1. Licensed by the Ontario College of Pharmacists (Part A)
2. Baccalaureate of Science Degree in Pharmacy or Doctor of Pharmacy degree
3. Minimum 2 years\xe2\x80\x99 experience working within a pediatric hospital preferred
4. Hospital Pharmacy Residency preferred
5. Demonstrated computer skills including pharmacy systems, word processing, spreadsheet and e-mail systems
6. Demonstrated ability to function as an independent practitioner
7. Demonstrated innovation and project leadership skills
8. Demonstrated commitment to continuous learning
9. Demonstrated excellence in independent practice, teamwork, change management, teaching, problem-solving and communication skills
