Clinic Locations
We currently operate two well-established clinics in Scarborough and Richmond Hill. Beginning in Fall 2025, we will be expanding with five new locations to support continued growth and community needs.

Employees will be assigned to a location that aligns with their preferences and residence, with occasional rotation to support clinic operations.
We are currently seeking a
Pharmacist
to join our Family Health Team.
Terms:
Full Time Permanent / Part Time Permanent
Responsibilities:
Provide education on pharmacological therapies, promoting adherence and understanding of treatment plans.
Offer evidence-based consultative advice to patients and team members on medication management.
Support medication reconciliation in collaboration with local pharmacies and healthcare agencies.
Manage complex medication regimens for acute and chronic conditions, (e.g. warfarin and smoking cessation therapies)
Identify and mitigate potential drug interactions or compliance challenges.
Collaborate with the team to develop, implement, and evaluate care plans.
Act as a drug-related information resource, providing updates on guidelines, safety alerts, and pharmacotherapeutic advancements.
Contribute to the development of clinical programs that enhance patient outcomes.
Drive quality improvement initiatives to optimize care processes and outcomes.
Maintain accurate and comprehensive documentation in the EMR system.
Requirements:
Current Pharmacist licensure (Part A) in good standing with the Ontario College of Pharmacists required
Board Certified Geriatric Pharmacist certification an asset
Additional credential and training with respect to dementia care an asset
At least 2 years recent clinical experience in the health care sector, preferably with geriatric experience
Use clinical judgment to identify and prioritize complex problems and provide suggestions to improve the patient's quality of life.
Knowledge of community resources for the provision of health promotion and disease prevention related services and chronic disease management
Experience in developing, implementing, monitoring, and evaluating health promotion programs and activities preferred
Sensitivity and awareness of cultural, racial, economic, and socially diverse communities of various ages
Strong sense of ethics and the ability to handle sensitive or private information with tact and discretion
Excellent assessment, analytical and problem solving skills
Excellent client service skills
Excellent interpersonal and communication skills (i.e. verbal and written)
Flexibility to meet the changes/demands of the patients/team/organization
Demonstrated ability to facilitate groups
Proficiency in Windows and Microsoft Office Suite
Good organizational and time management skills
Valid CPR/first aid certification
Ability to speak a second language such as Cantonese and/or Mandarin is an asset
A valid vulnerable sector police record check completed within one year
