The Clinical Pharmacist works as part of a collaborative team to provide medication support and health promotion services to patients of the PrimaCare Community Family Health Team.
Key Responsibilities
General:
Participation in chronic disease management & patient education
Adherence to PCCFHT policies and practices
Participation in continuing education events for the FHT
Participation and promotion of quality improvement initiatives
Communication with community agencies, as required
Specific:
Conduct medication reviews and assessments for patients with complex needs or comorbidities
Participate in collaborative medication reviews with patients and FHT team members
Act as a resource for prescribers and other interprofessional team members at the FHT
Monitor INRs and manage warfarin doses under FHT Medical Directive
Provide smoking cessation education and medication management under FHT Medical Directive
Document direct and indirect patient care in the EMR
Support a process of system analysis to improve seamless care during patient transition from hospital to home or long-term care
Develop a working relationship for medication reviews with FHT partners such as local hospitals, Ontario Health atHome, and long-term care facilities
Other duties as assigned
Position Requirements
Education:
Doctor of Pharmacy (PharmD) degree
Experience
:
Minimum 2 years
Professional Affiliations/Membership
:
Current registration with the Ontario College of Pharmacists
Other Skills/Abilities/Requirements
Recognize the limitations of their own experience and when to consult or refer elsewhere
Interest in professional development and maintaining knowledge of current best practices
COVID-19 vaccination required
Police records check, including Vulnerable Sector Screen to the satisfaction of PCCFHT
Physical/mental Demands
Walking, standing, sitting (regularly for extended periods of time)
Ability to shift concentration from one activity to another on a regular basis
Minimal lifting, pulling, pushing, stretching or physical exertion
