Pharmacist will ensure that treatment of the older individual is rational, safe, cost-effective and aligned with the person's healthcare goals and preferences. Geriatric pharmacist will support the patients and the team by ensuring safe and evidence based care throughout their stay, performing medication reviews, deprescribing medications that are no longer safe, etc'.
