Job Overview
We are seeking a compassionate and dedicated Support Worker to join our team. The ideal candidate will provide essential support to individuals in various settings, including assisted living facilities, home care environments, and hospice care. This role requires a strong commitment to enhancing the quality of life for those we serve, particularly individuals with disabilities and dementia. As a Support Worker, you will play a crucial role in delivering patient-centered care and ensuring the well-being of our clients.

Responsibilities - Live-in:

• Arrive Sunday evening and depart Friday evening. In addition to assigned Care Plan, available on-call as client requires.
• Assist clients with daily living activities, including personal hygiene, grooming, and meal preparation.
• Medications reminder as prescribed and monitor clients for any side effects or changes in condition.
• Provide companionship and emotional support to clients, fostering a positive and encouraging environment.
• Maintain accurate records of client care and progress, ensuring compliance with medical terminology standards.
• Assist with mobility and heavy lifting when necessary, ensuring the safety and comfort of clients during transfers.
• Collaborate with healthcare professionals to develop individualized care plans tailored to each client's needs.
• Provide specialized care for individuals with dementia or other cognitive impairments, utilizing effective communication strategies.
• Participate in ongoing training and development to enhance caregiving skills and knowledge of best practices in patient care.

Experience

• Previous experience in assisted living or home care settings is a must.
• Familiarity with medication serving protocols and medical terminology is highly desirable.
• Experience working with persons with disabilities or providing hospice care is an asset.
• Strong interpersonal skills and the ability to build rapport with clients from diverse backgrounds are essential.
• A compassionate approach to caregiving, coupled with a commitment to maintaining client dignity and respect.
