Job Summary


This is a permanent position

. Based in our Central Office, the analyst drives measurement and evaluation of continuous quality improvement activities in Palliser Primary Care Network (PCN) health homes and contributes to the PCN's overall evaluation strategy. This position encompasses a broad range of improvement/change leadership skills, including knowledge transfer/translation, facilitation and collaboration skills, to increase the efficiency, credibility and sustainability of the improvement measurement process.

The successful candidate builds strong relationships with PCN employees, clinic staff, EMR vendors and provincial peers, increasing the PCN's capacity to provide credible and consistent feedback and information.

Responsibilities

:

1. Primary Care Network Measurement and Evaluation

Drives the measurement of key access and clinical indicators to identify areas for improvement in efficiency and effectiveness of patient care, based on best evidence. Contributes to the PCN's evolution of its evaluation strategy. Researches, tests, refines, and presents methodologies to support the collection of key indicators. Becomes expert at defining, collecting, utilizing, analyzing, and interpreting operational and clinical measures. Collaborates with PCN clinics and other organizations (e.g. AH, AHS, AMA, HQCA) to implement and support interpretation of quality measurements. Creates summary documents and reports as requested by the PCN PMHO Evaluation Manager. Recommends improvements to the PCN's evaluation/measurement feedback process for optimized efficiency and effectiveness.
2. Office Practice Improvement Measurement

Collaborates on each health home team's ongoing panel identification and maintenance activities, to support optimizing family practice panel sizes that balance supply and demand. Collects, summarizes and interprets clinical data to support practice improvement, suggesting outcome, process and balancing measures that align with improvement goals. Recommends improvements to existing processes based on literature reviews and local tests of change. Develops and interprets summary reports for PCN clinics as required. Acquires an excellent working knowledge of the electronic medical record systems used by PCN clinics and becomes a trusted advisor to the PCN Administration Office team with an objective of increasing team EMR literacy. Extracts and merges data sets from multiple sources to support the PCN's evaluation for improvement and accountability. Communicates with electronic medical record representatives to improve systems, to improve analytics and health home usability. Refines processes to support the secure exchange of data.
3. Data and Technology Support

Assists with information technology equipment troubleshooting, evergreening, procurement, and maintenance (e.g. phone system, server, workstations) for PCN Administration Office. Supports the technology needs of PCN by providing technical assistance to computer system users in the PCN Administrative Office with an objective of increasing team IT literacy. Ensures success of PCN education events (e.g. workshops, staff meetings) with information technology planning, debriefing and event support as required.
4. Education

Conducts orientation and continuing education of PCN staff in relation to PCN measurement and evaluation. Acquires and maintains expertise in measurement and evaluation. Using adult learning principles, interprets technical jargon for a non-technical audience.

Requirements:



Education: A 3-year diploma or degree in science, technology, engineering, math or a related field is preferred. Experience: Data collection, analysis, visualization, data literacy education and/or evaluation experience is an asset. Culture: Palliser PCN is a continuous learning organization. A valid Alberta driver's license and a vehicle in good working order are also requirements for this position. Must have a recent (within the past three months) criminal record check/police information check (including vulnerable sector search) prior to the first day of hire.
