Schedule would be Monday to Friday 0800-1430 and every 2nd Saturday night 2300-0700

Overview

We are seeking a compassionate and dedicated Community Nurse join our team. In this role, you will provide high-quality nursing care to our pediatric clients in their homes, ensuring they receive the best possible support for their health needs. The ideal candidate will have experience in acute care and pediatrics, demonstrating a strong commitment to patient care and effective case management.

Responsibilities

Deliver comprehensive nursing care to patients in home settings, focusing on individualized treatment plans. Conduct thorough assessments of patients' health status and develop appropriate nursing interventions. Collaborate with multidisciplinary teams to coordinate patient care and ensure continuity of services. Educate patients and their families on health management, medication administration, and home care practices. Monitor patient progress and adjust care plans as necessary based on clinical evaluations. Maintain accurate documentation of patient care activities using electronic health record systems, such as Epic. Provide support for laboratory tests and procedures as needed, ensuring adherence to safety protocols.

Experience

Previous experience with pediatrics is highly desirable. Strong background in patient care with a focus on community home health Excellent communication skills and the ability to work collaboratively with healthcare teams and families. Experience working with children and understanding their unique healthcare needs is beneficial.
Training is provided.

Pay: From $32.00 per hour

Expected hours: No less than 32.5 per week
