Your Opportunity:

The Microbial Transplant Clinic (MTC) patient navigator is responsible for the coordination of patients referred to the Calgary Zone Microbial Transplant program. This position serves as a liaison between the program resource and referring physicians within the Calgary Zone. Decisions made by the patient navigator are largely based upon written information that is provided wither by referring MD/NP or patient information available on Connect Care or Netcare and are not based on direct patient assessment. Therefore, a strong general medical background, excellent communication, interpersonal skills, and the ability to be self-motivated and organized are required to ensure success in this role.

Description:

Determining the patient has indication and meets criteria for microbial transplant. Determining the patient does not have significant co-morbidities that would render them ineligible for microbial transplant. Be able to provide alternative recommendations to the referring MD/NP for alternative care pathways if patient is not eligible for a microbial transplant. Serve as a resource for referring MD/NP as it pertains to patient eligibility. Determination of patient follow up intervals. Patient and donor blood draw for screening and patient history. Assisting in and supporting additional research in the field of microbial transplant.

Required Qualifications:

Health focused degree with a strong medical background and demonstrated experience to meet the requirements of the position. Basic Cardiac Life Support - Health Care Provider (BCLS - HCP) required. Microbial Transplant experience and knowledge required. Minimum 5 years\' experience working in a health-related research field. Specific knowledge in clostridium difficile infection, antibiotic resistant gastrointestinal infections, and inflammatory bowel disease required. Demonstrated strong organizational, communication, and analytical skills. Ability to function independently, and within a team in a fast paced, dynamic environment. Additional Required Qualifications:

Excellent communication and decision-making skills. Professional manner with a focus on ensuring a very positive patient experience. Must be able to multi-task and problem solve. Intermediate computer skills required: MS Office, Connect Care, Netcare, research repositories. Completion of basic blood draw course or equivalent experience completing blood draws for research purposes only. Preferred Qualifications:

Master\'s degree preferred. Other related experience in facilitating groups, principles of adult education, preparing educational materials, conducting presentations, and managing change would be highly desirable assets.
