DESCRIPTION:
The Patient Care Navigator provides continuous support and advocacy for patients and their families from initial assessment through to community engagement. This role is designed to support, coordinate and enhance the medical and therapeutic treatment provided by a range of services in the acute hospital setting.
Within this role the Navigator is accountable for contributing to the delivery of the Kingston Health Sciences Centre strategy. As an employee, one must demonstrate an awareness of and be responsible for actively promoting and supporting patient and family centered engagement and care in all we do.
PRIMARY RESPONSIBILITIES & DUTIES:
Patient Care
Monitor and support the patient's journey from admission to discharge and transition into the community.
Communicate with patients, families and members of the interprofessional team to facilitate decision-making regarding care and transition to the community.
Interact with patients and families to address questions or concerns they may have related to their care plan and their transition through and from the hospital.
Collaborate with patients and their families and the interprofessional team to optimize recovery of the patient to the highest level of wellness possible, within an acute care setting.
Obtain, monitor and document information to support a collective understanding of the care plan between the patient, family and the interprofessional team.
Participates in the full scope of practice of the Registered Nurse, including provision of care as required.
Liaison
Facilitate and monitor the coordination and mobilization of health care and community resources to meet the needs of patients and their families. Upon collaboration with the interprofessional team members, this may include referral and consultation. Assess and monitor the continuity of care in order to expedite and coordinate the delivery of services to facilitate the patient's progress throughout the continuum of care.
Education
Assist patients and families in understanding their diagnosis and treatment options.
Participate in the development of education materials for patients and families in the Mental Health Program.
Participate in interprofessional education opportunities to increase knowledge and promote collaboration.
Program Support
Participate in continuous improvement opportunities to address wait times, barriers to care, patient and family concerns and/or issues related to patient flow.
Support and participate in ongoing program development, analyzing opportunities for improvement in the system and influences changes to maximize value patient whilst minimizing waste.
Interpret and use the results of the patient satisfaction survey in order to report on the patient experience for continuous improvement initiatives.
BASIC QUALIFICATIONS:
Registered Nurse (Current member in good standing with a College of Nurses of Ontario (CNO)).
CNA certification in Mental Health is an asset.
Minimum 3 years of recent related experience in Mental Health or Emergency, within the last 5 years required.
Enjoy working with patients who have mental health challenges to support and guide them in their recovery.
Demonstrated ability to educate staff, patients and families.
Demonstrated knowledge of community and hospital based resources available to patients and families.
Demonstrated ability to perform patient and family needs assessments and the ability to facilitate access of appropriate resources.
Demonstrated critical thinking and decision making skills.
Demonstrated ability to interpret/participate in research as evidenced through best practice with the patient population.
Demonstrated leadership skills within an interprofessional team.
Excellent interpersonal, communication, presentation and documentation skills.
Skill in organizing resources, analyzing and solving problems, establishing priorities and making evaluative judgements.
Demonstrated ability to work both independently and collaboratively with all team members.
Ability to manage complex situations.
Ability to take ownership of all aspects of a patient's discharge plan.
Evidence of strong mentoring, teaching, team building, change leadership, organizational and prioritization skills, with the ability to work well in stressful situations.
Demonstrated ability to effectively deal with conflict situations and proactively and effectively resolve issues impacting on quality and continuity of care.
Exceptional time management, organizational and prioritization skills.
Demonstrated ability to problem solve and make evidence based decisions.
Demonstrated knowledge of computer database, data analysis and presentation packages required.
