The Leduc Beaumont Devon Primary Care Network (LBDPCN) is a joint venture between participating local family physicians and Alberta Health Services. Our mandate is to sustain and enhance the quality, coordination, and integration of health care services in Leduc and area and to improve the quality of life for the community of patients and primary care providers.
We are seeking a dedicated and detail-oriented Panel Manager to join our collaborative team. The Panel Manager plays a key role in supporting proactive, team-based primary care by ensuring accurate patient panel management, data integrity, and care coordination. This position works closely with family physicians, nurse practitioners, clinic staff, and LBDPCN teams to improve patient access, preventative screening, and chronic disease management.
Duties and Responsibilities:
Follow patient privacy guidelines outlined by the Health Information Act (HIA).
Build strong, trusting relationships with primary care providers, clinic teams, and PCN staff to foster collaboration and shared understanding.
Implement best practices for panel identification, cleaning, and maintenance within member clinics.
Review and validate electronic medical record (EMR) data to ensure accurate patient attachment, demographics, and continuity of care.
Monitor and reconcile Community Information Integration & Central Patient Attachment Registry (CII/CPAR) reports to maintain accurate patient panel information.
Identify patients overdue for care, screening, or chronic disease follow-up using EMR reporting tools.
Coordinate proactive patient outreach and document all communication accurately within the EMR.
Work with the Quality Improvement Coordinator to support clinics in achieving quality improvement goals related to patient access, screening, and chronic disease management.
Requirements:
Diploma or degree in a healthcare-related field (e.g., Medical Office Assistant, Health Information Management, Health Sciences, or related discipline).
Minimum three years of experience in healthcare or primary care settings.
Experience working with EMRs (experience with multiple EMRs preferred).
Knowledge of panel management principles.
Valid Class 5 Driver's License and access to reliable transportation - travel between clinic sites is required.
Salary, Benefits, and Hours of Work:
The salary range for this position is $26.14 to $32.75/hour.
We offer group benefits, including a health spending account.
Working hours will be scheduled Monday to Friday, from 8:00 a.m. to 4:00 p.m. (some flexibility required), with a 7.5-hour workday.
