Visit us today and check out to see why more people from across the globe are moving here.About the OpportunityReporting to the Health Services Manager, Palliative Navigator, through an intake process, identifies clients/family needs, develops recommendations, and evaluate the most appropriate services and programs that will assist in meeting the frail elderly/complex case individual/family\'s goal in living in their own community for as long as possible. Works collaboratively with the client\'s family physicians, multi-disciplinary health care team and community supports/agencies. Supports access to the services and programs that enables successful pain for the client/family.
