We are currently hiring for 1 Part-time Outreach Worker to work 27hr/week.
Job Summary:
The Outreach Worker is responsible for working in a mobile capacity providing community-based support, housing and service navigation in order to support people who are unhoused and may experience mental health and substance use concerns. The team will work within the community meeting people where they are at, including street level outreach. Reporting to the Outreach Manager, or such other persons as required by the organization, the Outreach Worker must always operate with a 'safety first' mindset and engage in a collaborative approach as they support unhoused community members who are not currently staying at The Lighthouse.

Responsibilities:
Participant Relations
Use case management as a tool to support participants in identifying their unique housing barriers, set goals to achieve, and move toward housing.
Provide participants with basic necessities.
Use a trauma-informed and non-judgmental approach to support and encourage participants in reaching housing and related goals.
Complete accurate, professional, factual and non-judgmental incident reports, participant records and case management notes.
Help to build professional and positive relationships that are healthy and supportive.
De-escalate and problem-solve with participants and help them work through the issues that are causing challenges.
Provide participants with supplies and education related to harm-reduction as appropriate.
Community Resources and Supports
Assist participants to connect to appropriate community resources (emergency services, mental health and other crisis supports, etc.).
Collaborate with community partners to explore unique solutions to meet participants' diverse needs.
Safety
Conduct outreach services while maintaining safety for self, other staff and participants as the top priority.
Record and report any safety concerns promptly.
Call management on duty if a serious occurrence occurs.
Reporting
Complete HIFIS reporting on all pertinent data which includes but is not limited to intake, VI-SPDATS, housing history, case notes and services offered.
Complete Lighthouse documentation as required using Google Docs and Google Sheets.
Assist with the reporting on supplies for purchase and ordering new products as needed and as directed.
Perform other duties as assigned.
Qualifications
Diploma in Social or Human Services or equivalent education experience.
1 year of experience working with those experiencing homelessness and providing housing assistance within a diverse community.
Lived experience is an asset.
Valid Ontario driver's license.
Must have a clean driving abstract and be minimum age 25 for insurance purposes.
Clean criminal record check, including vulnerable sector screening.
NVCI, First Aid CPR, and other related trainings considered an asset?.
High standard of integrity, judgement and confidentiality, with a passion to end homelessness and be a good role model.
Demonstrated ability to multitask, prioritize and organize.
Demonstrated conflict resolution, advocacy skills, understanding of harm reduction, trauma-informed support skills and a person-centred approach.
Adequate administrative, documentation, and computer skills, including Google Docs, Google Forms, Google Sheets, Excel, etc.
Effective communications skills, both written and verbal.
Excellent customer service, problem-solving and decision-making skills.
Flexible and adaptable with the ability to work independently and as part of a team.
