Position Status: This is a Bargaining Unit position. All terms and conditions of the bargaining agreement apply.

35 hours per week (1-2 evenings per week, occasional weekend work)

Salary Range: Annualized salary range $50,191 - $60,542, commensurate on skills and experience plus Group Benefits & HOOPP



Primary Work Location:

Durham Region and at all DCHC sites

Durham Community Health Centre (DCHC) is a registered, charitable organization that provides integrated, accessible, and equitable community-based primary care, wellness services, and health education to Durham community members who face multiple barriers to their health and well-being.





DCHC also provides equity-based programs and services that focus on priority populations such as Indigenous, Black, the 2SLGBTQI Community, Newcomers to Canada, Seniors, and Unattached patients (i.e., those without a family doctor), to name a few. DCHC ensures Durham community members receive not only sick care, but preventive care as well.



At DCHC, we are a team-based interprofessional group of staff including physicians, nurse practitioners, nurses, counselors, dietitians, outreach workers, medical secretaries, and other administrative staff. We always place every client at the centre of our approach to care, based on their needs. We address these needs through integrated clinical and wellness care and health education.

Position Overview




As a member of an interdisciplinary team, the Outreach Worker will deliver comprehensive, client-centered and addiction-focused health services to individuals experiencing homelessness and/or substance use challenges in the community.



The Outreach Worker will work collaboratively with internal and external providers to ensure integrated, wraparound supports aimed at improving access, reducing barriers, and addressing social determinants of health. The Outreach Worker will refer clients to community- based and specialty services as indicated.



The Outreach Worker will demonstrate advocacy and leadership skills to champion and support goals that align with and achieve DCHC's Strategy, Vision, Mission and Values, internally and externally.

Key Responsibilities



Outreach Services



Provide a range of Outreach support services, including short and long-term case management that promotes wholistic wellbeing for those experiencing homelessness, at risk of homeless or in a crisis. Provide financial aid to those needing short term financial support for personal services such as prescription medication costs, physiotherapy services, transportation, food vouchers, etc. Provide a range of opportunities for clients to learn daily life and living skills, while increasing their sense of community and belonging and connection to culture and traditions as necessary. Provide comprehensive culturally appropriate resources and culturally relevant service plans. Educate clients about their tenant rights and responsibilities. Contribute to the coordination, planning and enhancement of local services Support the implementation of realistic and culturally relevant goals and objectives through regular community and home visits. Act as an advocate on clients' behalf Access and utilize community resources for clients. Keep detailed client records to provide quality care. Coordinate and submit expense claims to financial services. Participate in events, workshops/orientations for clients and community. Coordinate, support and facilitate Indigenous based programming.

Administration, Quality Improvement and Reporting



Document within DCHC's EMR, information regarding outreach contacts. Complete Incident Reports as required. Follow policies and standards regarding confidentiality and documentation. Ensure all necessary reports are completed accurately and in a timely manner. Attend relevant team meetings and planning activities. Participate in quality improvement and evaluation activities. Work with the health and safety guidelines provided by the agency, which includes annual mandatory training requirements.

Key Qualifications



Completion of a formal 2-year post-Secondary program at a Community College (Health/Social Services). Minimum 1 years related experience or equivalent combination of education and experience Experience providing support services Current knowledge of community programs, services and resources Highly organized and compassionate Understanding and willingness to practice Covid-19 safety protocols Available to work a flexible schedule Valid driver's license is required, and regular access to a vehicle
