Job Purpose



The Outcomes Monitor will be accountable to managing database matters. This includes providing support to records assistants and staff in completing the full client care cycle. The assistant's duties include helping any practitioner assess, plan, implement, monitor, and evaluate actions required to meet the client's service needs.

Duties and Responsibilities



The Outcomes Monitor will:

Conduct follow-up surveys for client outcome measurement. Assist in file and data entry needs related to the staff client files. This includes using various internal to CFN and external to CFN databases and electronic or on-line databases or data depositories. Oversee the upload of files to I-CARE or other funder databases. Assist with the compiling and analyses and review of data files and client data for use in staff review of files for improvement of client services. Accomplishes clients' care by assessing client needs, developing, monitoring, and evaluating supporting plans and progress, facilitating interdisciplinary approaches, and monitoring staff performance. Assists with admitting new clients by reviewing records and applications and conducting orientations and intake assessments as required. Determines clients' requirements by assisting with interview intakes, determining the need for medical, psychosocial, and psychiatric evaluations or other needs related to client objectives, and service plans. Maintains clients' records by reviewing case notes and logging events and progress. Prepares reports by collecting, analyzing, and summarizing results data and trends; compiling statistics; assisting grant and subsidy applications. Completing other duties related to outcomes and database management as requested. Contributes to a positive, culturally sensitive working environment.

Requirements



Diploma or degree in related social services or education field Proficient in Microsoft Office Suite, especially Excel and advanced features of database software. Some experience with immigration, refugee, or related issues is highly preferred Previous experience with psychological aspects of care Effective communication skills Excellent organizational and time management skills Familiarity with professional and technical emerging knowledge Problem-solving skills and ability to multi-task Compassionate with teamwork skills Certificate of completion from attending the IRCC-GBA Plus course
