Program Profile
Ontario Lung Screening Program is a screening program for people at high risk of getting lung cancer. The referral inclusion criteria for participants are those between the ages of 55 and 74 years old, and who have smoked cigarettes every day for at least 20 years.
Position Summary
In conjunction with the Firestone Ambulatory Clinic (Respirology and Thoracic clinics), the Ontario Lung Screening Program (OLSP) Coordinator Nurse, as a member of the Ontario Lung Screening Program, will provide patient-centered system navigation, screening, case management, and coordination of services to patients who are referred to the program. The OLSP Coordinator Nurse will identify and facilitate access to the right services from the right provider at the right time based on assessed needs by working collaboratively with an interprofessional team. The successful candidate acts as a primary resource for the SJHH Lung Screening program. The Coordinator Nurse will also provide support to the Firestone Ambulatory Clinic as required. OLSP Coordinator Nurse has excellent communication and leadership skills such as; problem-solving, critical thinking, negotiating, and conflict resolution. This role is also responsible for establishing and maintaining relationships with other healthcare professionals in designated specialty functions to ensure quality of care.
The OLSP Coordinator Nurse will coordinate seamless care transitions from referral, triaging protocols, lung screening, and ongoing management plans as needed. Responsible for exchanging knowledge related to screening results and the continuum of services or care required for each patient. Determine eligibility for lung screening and follow the patient through the screening journey in engagement with community healthcare providers. The OLSP Coordinator Nurse will also act as a liaison between patients and providers of other Community/Clinical Support Services. OLSC program leverages cutting-edge technology to support patient care with seamless transitions from screening to ongoing community management.
The OLSP Coordinator Nurse will be responsible for ensuring cost-effective patient care by utilizing resources efficiently and collecting necessary data on client care to support the evaluation of outcomes.
Qualifications
Current RN registration with the College of Nurses of Ontario required
3 years' experience with intake functions, community relations and service development
Three (3) years of recent clinical experience in a Community/Ambulatory health setting is required; experience in working in an acute care setting is an asset
2 years' experience with Respirology /Thoracic patient population preferred
Demonstrates ability to work independently and collaboratively as a part of a multidisciplinary team
Aseptic Technique and experience with minor procedure set-ups and assisting with procedures
Experience with venipuncture, intravenous therapy, administering medication, oral, IM or IV route, allergy skin testing, removing sutures and clips, etc. required
An enthusiasm for quality improvement in healthcare
Proven skills in problem recognition and problem-solving
Compliance with confidentiality requirements
Knowledge of applicable legislative, SJHH, and departmental policies
Comprehensive knowledge of community resources and health care delivery systems in acute, and community settings
Demonstrated ability to respond to patients and situations with flexibility and adaptability; experience in performing case management functions is an asset
Demonstrated expertise and competence in areas of patient care, including chronic disease management, mental health, and addictions, medication, and symptom management for specific diseases
Excellent interpersonal, communication, organizational, and decision-making skills required
Demonstrated leadership skills, including problem-solving, critical thinking, conflict resolution, and negotiation
Demonstrated ability to work independently and collaboratively with an interdisciplinary team, family members, support networks, and a variety of formal and informal service providers
Excellent time management and stress management skills required
Experience in coordinating patient care in an ambulatory setting
Previous experience in patient counseling
Knowledge of smoking cessation counseling techniques is an asset
Ability to perform duties professionally and courteously and produce high-quality work while meeting deadlines in accordance with SJHH standards
Demonstrated knowledge of clinical pathways and Continuous Quality Improvement (CQI)
Effective communication skills, both verbal and written
Computer literacy and basic proficiency is required
A valid driver's license, adequate insurance (minimum of $5 million personal liability), and use of a reliable vehicle required
