About the Opportunity

Please note: This position requires the successful incumbent to strictly adhere to Integrated Chronic Care Services (ICCS) scent-free free policy.

The ICCS provides comprehensive assessments and care planning for individuals with complex chronic conditions and multi-morbidities such as: environmental illness, chronic fatigue syndrome, fibromyalgia, functional neurologic and gastrointestinal syndromes.

The ICCS model is based on the premise that chronic disease outcomes are better managed by addressing the needs of the whole person with a focus on self-management support, with a particular focus on behaviour change. With a team of interdisciplinary health care professionals, we integrate the physical, psychosocial and environmental needs of individuals by focusing on the person instead of the disease(s)/condition(s), improving functional health and quality of life. Our care team includes: physicians, occupational therapists, a nurse practitioner, registered nurse, licensed practical nurse, psychotherapist, and a dietitian with support from an admin team.

The Occupational Therapist (OT) works as an interdisciplinary team member in collaboration with patients, caregivers and families to facilitate, restore and/or maintain patients\' optimal level of function in the areas of self-care, productivity and leisure. Responsibilities of the Occupational Therapist (OT) include:

  • provide individualized OT assessment, consultation, treatment, evaluation, and follow-up.
  • collaborate with an interdisciplinary team to develop comprehensive care plans to support optimal functional health and quality of life for patients based on patient identified goals.
  • provide care coordination to ensure seamless care delivery and monitor/evaluate team-based care plans.
  • develop/facilitate group-based programs to address self-management and functional well-being of patient population.
  • manage a busy caseload and use clinical judgement and decision-making skills to prioritize issues on a daily basis.
  • possess excellent interpersonal and written communication skills are required.
  • adhere to NSHA policies and procedures and to the professional standards of our licensing body, the College of Occupational Therapists of Nova Scotia (COTNS).
  • contribute to ongoing quality improvement initiatives to achieve best possible patient outcomes and stewardship of health system resources.
  • participate in the support and mentorship of student learners.
About You

We would love to hear from you if you have the following:
  • Graduate of a Bachelors Degree in Occupational Therapy from a recognized School of Occupational Therapy
  • Registered with the College of Occupational Therapists of Nova Scotia (COTNS)
  • BCLS certification
  • Demonstrated effective communication and interpersonal skills
  • Must be willing to strictly adhere to scent-free policy of ICCS
  • Skills/experience in motivational interviewing and behaviour change theory
  • Competencies in other languages an asset, French preferred

Hours
  • Permanent Part-time (50%) position; 37.5 hours bi-weekly

Compensation and Incentives

$33.97 - $42.89 Hourly
