The Interdisciplinary Clinical Leader works closely with patients and families, team members (physicians, psychologists/clinical therapists, occupational therapists, physiotherapists, social workers, nurses, office staff) family physicians, community organizations, primary, secondary and tertiary health services, to promote person-centered collaborative care within the Primary Health Care portfolio.The incumbent develops and fosters a healthy interdisciplinary team processes that develop a person and family centered continuum of care focused on functional health goals, building relationships with community based services ensuring practice is interdisciplinary and evidence based, program planning and evaluation, participating in the recruitment, selection and orientation of new team members, supporting a quality approach to care delivery, coordinating and providing clinical care to patients.About YouWe would love to hear from you if you have the following:
