Occupational Therapist - Independent contractor (Part time; Full-time)

As a Community Home Health Occupational Therapist, you will play a vital role in empowering clients to achieve their highest level of functional independence. This role will focus on supporting patients with complex needs by conducting in-home assessments, developing individualized treatment plans, and delivering therapeutic intervention. Your expertise will contribute to enhancing their quality of life and promoting overall well-being.



Responsibilities:



Conduct comprehensive assessments of clients' functional abilities and limitations. Develop individualized treatment plans based on assessment findings. Provide evidence-based occupational therapy interventions to improve clients' mobility, self-care, and daily living skills. Collaborate with other healthcare professionals, caregivers, and family members to ensure holistic care. Educate clients and their families on adaptive techniques and equipment. Maintain accurate documentation of assessments, progress, and outcomes.

Qualifications:



Bachelor's or Master's degree in Occupational Therapy Current licensure as an Occupational Therapist in Ontario Minimum of 12 months experience in home health or community-based settings preferred Strong communication and interpersonal skills. Compassionate, patient-centered approach.
