Private In-Home Care (1:1 Client-Centered Support)
About the Role:
We are seeking a compassionate and experienced
Occupational Therapist (OT)
with expertise in
dementia care and cognitive stimulation
to support a 78-year-old client with early mild dementia. The client lives at home with family, is fully independent with basic activities of daily living (ADLs), but requires assistance and structured support to manage instrumental activities of daily living (IADLs) and cognitive changes.
This is a unique opportunity to provide
one-on-one in-home therapy
focused on preserving independence, enhancing cognitive engagement, and improving quality of life in a supportive, family-centered environment.
Key Responsibilities:
Conduct in-home assessments to evaluate cognitive function, daily routines, safety, and adaptive needs.
Develop and implement personalized therapy plans that include:
Cognitive stimulation activities
Memory-enhancing exercises and tools
Functional strategies to support IADLs
Provide caregiver education and training to family members on communication strategies and daily support techniques.
Recommend and introduce adaptive equipment, memory aids, and environmental modifications.
Monitor progress and adjust treatment plans based on evolving needs.
Ideal Candidate:
Licensed Occupational Therapist in Ontario
Minimum 2 years of experience in geriatrics, memory care, or cognitive rehab
Strong understanding of
mild cognitive impairment (MCI)
and early-stage dementia
Skilled in delivering person-centered, in-home therapy
Compassionate communicator with a collaborative mindset
Experience with caregiver coaching is a plus
