We have an exciting new opportunity for a regular full-time Occupational Therapist (OT) role in the Integrated Community Program, working in the two GIM units of C6 and C4. The patient populations on C6 include short stay GIM, plastic surgery and gynaecologic oncology, where there is rapid turnover of these beds. The patient population on C4 is GIM. The OT will be responsible for patient assessment, intervention, and recommendations that facilitate effective discharge planning. The OT will collaborate with patients, families and the interprofessional team members to identify and support the patient's occupational performance goals for care and to optimize safety in the patient's transition out of acute care. The OT will participate in profession-specific activities of Occupational Therapy, including annual student education activities in support of the Department of Occupational Science and Occupational, as per Sunnybrook's full affiliation with the University of Toronto.
You will act as an occupational therapy expert resource for staff, students, and other healthcare providers.
