Position Summary
:
The Nurse Practitioner (NP), Cardiac Surgery, is a Registered Nurse in the extended class. The NP functions at full scope of practice demonstrating a high level of autonomy and expert skill to formulate clinical decisions and appropriately manage acute/chronic illness and promote wellness within the NP's area of specialty. The NP utilizes a comprehensive theoretical knowledge base and an advanced level of clinical competence in caring for patients. This involves health assessment, diagnosis and therapeutic management of individuals. The NP consults and collaborates with individuals, their families, and the interdisciplinary team. The NP performs research, influences clinical practice, and provides leadership within the organization.
In addition, the NP, with a specialized focus on cardiac surgery, uses their expert skill to formulate clinical decisions and appropriately manage patients independently and in collaboration with members of the health care team. New and experienced NPs are welcome to express their interest for this position. A robust orientation and mentorship are provided for new NP hires.
Responsibilities:
+ Direct Comprehensive Care: common patient issues include arrhythmias and stroke risk assessment, heart failure management, and pre and post op assessments and management.
+ Performs and documents a comprehensive health history and physical examination. The assessment includes psychosocial, spiritual and cultural assessments that may impact patient care needs.
+ Identifies and orders diagnostic tests and procedures within the scope of their approval licensure and according to the established medical directives.
+ Collects, interprets and analyzes data from multiple sources and uses this data to formulate a medical diagnosis.
+ Develops, implements and documents the plan of care for the patient.
+ Prescribes pharmacological and non-pharmacological therapies according to approved license.
+ Evaluate and revises plan of care based on response of patient/family.
+ Coordinates the interdisciplinary plan of care for patients by liaising with members of the interdisciplinary team to facilitate the efficient movement of the patient through the health care system.
+ Perform cross-functional and/or other duties within the job classification as requested or assigned.
Leadership
+ Provides leadership in the development, implementation, and evaluation of quality management programs.
+ Participates and provides leadership in the strategic planning process for the specific clinical program.
+ Participates in the development, implementation and evaluation of policies and procedures and care delivery models.
+ Facilitates the development of standard work, protocols and clinical pathways.
+ Participates on committees and task forces as appropriate.
Education
+ Contributes to the development/implementation and evaluation of patient education materials.
+ Conducts educational teaching sessions with patients and/or families with a focus on health promotion, disease prevention and impact of illness.
+ Facilitates professional development of staff through education.
Position Requirements:
Masters in Nursing.
Completion of a Nurse Practitioner Program recognized by the College of Nurses of Ontario.
Registered Nurse in the extended class with Ontario Certificate of Registration with the College of Nurses of Ontario without restrictions.
Member of Registered Nurses Association of Ontario or Member of the Nurse Practitioner's Association of Ontario with Legal Assistance Program (LAP).
Able to accept and provide constructive feedback.
Demonstrate ability to work independently with a high degree of self-direction and strong organizational skills; as well as working effectively as a member of an inter-professional team.
Demonstrate excellence in communication skills (written and oral) including conflict resolution.
Knowledge of adult education principles
Strong organizational skills including: time management skills, flexibility, the ability to prioritize among competing demands, and the ability to multitask.
Demonstrates ability to work in a collaborative practice model with inter-professional teams.
Demonstrated commitment to pursuing ongoing education.
Demonstrated ability to use initiative and work autonomously utilizing critical thinking skills, advanced decision-making, problem recognition, and problem solving skills.
Ability to apply best practice modalities for assessment and treatment within professional scope of practice.
Successfully able to perform their function in accordance with prescribed standards that focuses on the priority of People Safety and the delivery of the highest standard of Quality Care.
Demonstrate the physical and mental capabilities necessary for proper and efficient performance of the essential duties of the position.
Excellent and dependable attendance record with a proven teamwork attitude
Knowledge of an adherence to the Code of Conduct and all other workplace policies, including but not limited to Ethics, Violence and Harassment.
Knowledge and compliance with Occupational heath and safety act and it's regulations, reporting hazards, deficiencies and contraventions of the Act.
Preferred Requirements:
Three (3) - five (5) years' experience in cardiovascular surgical nursing Current Basic Cardiac Life Support (BCLS) certification or be willing to obtain within 3 months of hire
Current Advanced Cardiac Life Support (ACLS) certification or be willing to obtain within 6 months of hire.
Successful completion of a Basic Cardiac Arrhythmia and a ECG interpretation course or be willing to obtain within 6 months of hire
Knowledge or, and recent use of Cerner.
Knowledge of the principles of change management.
Canadian Nurses Association Specialty certification in Cardiovascular Nursing.
