Reports primarily to the Program Director, Chronic Disease Prevention & Management and Medicine Service and a
matrix reporting relationship to the Chief Nursing Officer/Professional Practice Leader, with accountability to the
Hospitalist Physician preceptor. The Physician preceptor will provide a partnership with the Nurse Practioner (NP), to
provide guidance when outside of their scope of practice, will validate the ability to perform controlled acts, assist in
the development of Medical Directives and provide input into annual performance appraisals. The NP is responsible
and accountable for his/her own practice.
The Nurse Practitioner (NP) within the Hospitalist Program works within the framework of nursing in collaboration
with the healthcare team to provide and enhance care for Adult orphan patient population. NP practice addresses the
five domains of Advanced Practice Nursing: Clinical Practice, Collaboration, Leadership, Research and Change
Agent. The time dedicated to each of these domains is variable based on the program needs.
Applicants must have the following qualifications: Successful completion of a Nurse Practitioner program (ACNP Program preferred).
Current RN registration with the College of Nurses of Ontario in the Extended Class (EC)
A minimum of 3?5 years of recent experience in acute care setting
In addition, applicants must have skill and experience in the following elements of nursing practice: Knowledge and experience in change theory, adult learning principles and health promotion
Effective patient and family counseling skills
Excellent interpersonal/communication skills
Strong organizational, critical appraisal and analytical skills
High level of initiative and self direction
Ability to work effectively both individually and as part of a team
Demonstrated coaching and mentoring skills
Proficient in computer applications and software such as Group Wise, Microsoft office, Powerpoint and Excel
An understanding of health care issues within the community and experience in health services research, and
program evaluation
Salary Range: $65.210 to 70.750
