Overview

We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will provide high-quality patient care This role involves working closely with patients to assess their health needs, develop treatment plans, and manage ongoing care.

Responsibilities

Conduct comprehensive patient assessments and physical examinations. Develop and implement individualized treatment plans based on patient needs. Administer medications and monitor patient responses to treatments. Provide education and support to patients and their families regarding health conditions and treatment options. Collaborate with healthcare teams to ensure coordinated patient care. Perform phlebotomy as needed for diagnostic testing. Manage care for patients with dementia and other chronic conditions. Participate in quality improvement initiatives within the practice.

Skills

Strong understanding of medical terminology relevant to patient assessments and documentation. Ability to work effectively in a medical office setting, demonstrating excellent organizational skills. Strong interpersonal skills to communicate effectively with patients, families, and healthcare professionals.
