JOB DESCRIPTION

The Kapuskasing and Area Family Health Team (KAFHT) is dedicated to providing high-quality, patient-centered primary care. As a Nurse Practitioner, you will be an integral part of our interdisciplinary team, working collaboratively with physicians and allied health professionals to deliver comprehensive care.

KEY RESPONSIBILITIES

Conduct comprehensive assessments, diagnose conditions, and create personalized treatment plans. Prescribe medications and manage chronic diseases using evidence-based guidelines. Order, perform, and interpret diagnostic tests. Provide patient education and counseling to promote health and prevent disease. Collaborate with physicians and other healthcare professionals to ensure coordinated care. Maintain accurate and timely documentation in patient records. Participate in program development and quality improvement initiatives. Other duties that are deemed appropriate and assigned by the Executive Director of the KAFHT.

JOB REQUIREMENTS

Master of Science in Nursing (MScN) - Primary Care Nurse Practitioner. Current registration as a Nurse Practitioner in good standing with the College of Nurses of Ontario. Strong clinical decision-making, communication, and critical thinking skills. Ability to work independently and as part of a multidisciplinary team. Bilingualism (English & French) is an asset. Proficiency with EMR (PS Suite) and computer software such as Outlook, Word, and Excel. Entrepreneurial spirit directed to helping the KAFHT in the delivery of programs.
