The eight Family Health Teams (FHTs) in Huron and Perth have collaborated to deliver a regional Primary Care Program aimed at offering primary care services to residents lacking access to such care. Established in October 2024, the team is comprised of Nurse Practitioners, nurses, and other healthcare disciplines, as well as administrative supports, to provide team-based primary care to unattached patients. This newly funded program is part of the Enhancing Interprofessional Care Teams investment in primary care. The program runs at multiple sites throughout Huron and Perth with one of the site locations within the Stratford Family Health Team (SFHT). SFHT is currently recruiting for a Nurse Practitioner to support this initiative.
Job Duties:
Provides direct comprehensive care to patients with specialized and complex needs.
Completes health assessments including health history and physical examinations.
Works within the legislated scope of practice of the Nurse Practitioner, determines the need for, orders and interprets reports of appropriate laboratory and diagnostic imaging.
Monitors the ongoing therapy of clients with chronic stable illnesses by providing pharmacological and counseling interventions.
Determines the need for, and the implementation of health promotion and prevention strategies for families and individuals.
Uses nursing strategies based on evidence-based information.
Formulates and communicate medical diagnosis or disorder.
Consults with and refers to other health care professionals and community services as needed.
Gathers and documents statistical information as required.
Documents all encounters on an electronic medical records system ensuring that data quality standards for charting are met.
Qualifications and Experience:
Current registration with and a member in good standing of the College of Nurses of Ontario in the extended class without restriction.
3-5 years' NP work experience preferred.
Current membership in the Nurse Practitioners Association of Ontario.
Primary health care experience, preferably in a community setting.
Advanced proficiency in the use of computers and various software applications including Microsoft Office and electronic medical records systems (experience with Telus CHR is an asset but not required).
Demonstrated excellent problem-solving, organizational, and interpersonal skills with an ability to work effectively and respectfully in a collaborative, interdisciplinary team environment.
Experience and willingness to provide care of geriatric populations.
Experience working with individuals with complex needs.
Strong interpersonal, communication and organizational skills.
Politically and culturally sensitive.
High level of critical and logical thinking, analysis and/or reasoning to identify underlying principles, reasons, or facts.
Experience working in a Primary Care Setting.
Compensation: Annual full-time salary is $130,330 (negotiable) and will be pro-rated based on the number of hours the candidate works.
Expected hours: The position is available for either a minimum 24 hours per week to a maximum of 32 hours per week (depending upon the candidates interest and availability)
