Position Summary:




The Nurse Practitioner (NP) will deliver comprehensive primary health care services to clients across the lifespan. The NP will emphasize health promotion, disease prevention, and the management of chronic and acute conditions. The role involves working collaboratively within a multidisciplinary team, incorporating traditional and Western approaches to health care, and fostering culturally appropriate care.

Key Responsibilities:



Provide primary health care services, including health assessments, diagnosis, treatment, and follow-up for clients of all ages. Manage chronic diseases such as diabetes, hypertension, and respiratory conditions, in collaboration with clients and caregivers. Perform procedures and diagnostic tests within the scope of practice. Prescribe medications and treatments according to current guidelines and regulations. Support mental health care through counseling, assessments, and coordination with mental health professionals. Integrate traditional healing practices and collaborate with Elders, Traditional Healers, and Knowledge Keepers. Deliver culturally sensitive care that respects Anishinaabe values and traditions. Lead community health initiatives focusing on prevention, education, and empowerment. Provide health education to clients, families, and the broader community to enhance self-care and overall health outcomes. Participate in health workshops, group sessions, and community events as required. Work within a multidisciplinary team, including physicians, traditional healers, mental health workers, and other health professionals. Refer clients to specialists, diagnostic services, or other programs as necessary. Participate in team meetings, case reviews, and collaborative care planning sessions. Maintain accurate, thorough, and timely client records using electronic medical record systems. Stay informed about best practices and developments in primary care, Indigenous health, and holistic health models. Participate in ongoing professional development, training, and cultural competency workshops.

Qualifications:



Master's degree in Nursing or equivalent Nurse Practitioner program. Current registration with the College of Nurses of Ontario (CNO) as a Nurse Practitioner (Primary Health Care). CPR and First Aid certification (current). Minimum of 2 years of experience as an NP in a primary care or community health setting is preferred. Experience working with Indigenous communities or in a cross-cultural environment is an asset. Strong understanding of Indigenous health issues, social determinants of health, and trauma-informed care. Excellent clinical skills with the ability to provide comprehensive and holistic care. Strong interpersonal and communication skills to build trust with clients and the community. Proficiency in electronic medical records (EMRs) and other clinical documentation tools.

Additional Requirements:



Knowledge of Anishinaabe culture, language, and traditions is an asset. Valid driver's license and access to reliable transportation. * Criminal record check with a vulnerable sector screening.
