The Nurse Practitioner (NP) - Family/ All Ages is an advanced practice role providing professional, analytical and an autonomous comprehensive range of health services to clients and patients including but not limited to health assessments and diagnosis diseases, disorders, injuries and conditions; treating and managing acute and chronic illnesses and identifying health risks and needs within a holistic model specifically within the vulnerable/homeless population.The NP will provide comprehensive care to clients using principles of primary health care utilizing a harm reduction model of care within a health promotion, population health and self-help framework. In the provision of comprehensive primary health care services the NP works with an interdisciplinary heath care team in a collaborative approach to care. The NP acts as a resource leader for their area of practice and participates in the development, implementation and evaluation of policies, procedures and protocols.The NP will provide quality nursing care and expertise to clients of Eastern Health\'s Primary Health Care & Chronic Disease Prevention and Management, vulnerable populations family care team. Practicing through a harm reduction lens, responsibilities include but are not limited to: assisting in the development and implementation of provincial harm reduction-focused programs, policies, learning opportunities, and awareness activities, liaising with other services such as Communicable Disease and Laboratory Services, seeing clients individually, and collectively where appropriate, e.g. groups education/learning, and provides support to the significant other(s) of those clients.Job Demands

Constantly moves, lifts, or operates equipment/machinery up to 10 lbs. and regularly is required to lift/move equipment, books, supplies, and sometimes children up to 25 lbs. Occasionally, may have to reposition a patient over 50 lbs.
Physical effort also includes regular sitting to conduct patient histories, performing work on the computer and when assessing patients, regularly stands or walks.
When performing procedures, regularly uses fine finger/precision work and hand tools (i.e. medical instruments, computer mouse, stethoscope, dopplers, etc.) requiring accurate control and steadiness.
Visual concentration is required when performing physical assessment and procedures, completing charts/reports, observing patients, reading equipment/monitors, and completing work-related correspondence on the computer.Job Qualifications
Completion of an approved/recognized Nurse Practitioner program is required.
Current registration with the College of Registered Nurses of NL and Licensed (or eligible for licensure) as a Nurse Practitioner (Family/All Ages) with the CRNNL.
A minimum of five years clinical experience within the last seven in Primary Health Care is required.
Clinical experience in mental health and/or addictions would be considered an asset.
Previous harm reduction experience specifically related to vulnerable, unhoused, and/or under-serviced populations would be considered an asset.
Current BCLS Certification is required.
Possesses strong abilities in the areas of collaboration, clinical reasoning, professional development, interpersonal effectiveness, conflict management and teamwork.
Demonstrates an understanding of the complexities of vulnerable populations and harm reduction principles.
Is self-motivated with a demonstrated ability to work collaboratively within an interdisciplinary team, and across multiple related programs.
Demonstrated progressive clinical leadership skills.
Possesses strong communication, problem solving, decision-making, and teaching skills.
Demonstrated ability to function autonomously in an advanced practice role.
Supports and participates in research to improve practice and client outcomes.
Demonstrated skills in computer and database management skills.
Due to the requirement to complete home visits and work across multiple sites, a valid Newfoundland and Labrador Class 5 Driver\'s License and the use of a private vehicle are conditions of employment.
