Job Summary
As a member of an interdisciplinary team, the Nurse Practitioner provides quality care to clients of the Family Care Team, providing services to residents in the St. John\'s Metro region, with potential service to alternate sites using a hub and spoke model. The principles of the Family Care Team align with the patient health home model of care, using a collaborative health care team approach, from various health disciplines, in the provision of primary health care services.
This position works within the Nurse Practitioner\'s (NP) scope of practice in the following category of practice: (Family/All Ages). This is an advanced practice role providing professional, analytical and an autonomous comprehensive range of health services to clients, including but not limited to, health assessments and diagnosis of diseases, disorders, injuries and conditions; treating and managing acute and chronic illnesses; and identifying health risks and needs within a holistic model. The NP in this position may complete home visits and travel to other primary healthcare clinics in the area for relief/support.
The Family Care Team mandate is to provide patients without a primary care provider of the St. John\'s Metro region with access to health care services. Services will be provided using an inter-professional collaborative approach where patients will see the right provider at the right time and right place. All health care providers, including NPs, will be expected to practice to their full scope of practice in order to meet patient needs and requirements. The Family Care Team aims to provide continuity of care to improve health outcomes and patient quality of life through patient attachment and patient paneling to individual NPs, with the support and wrap around services of the full team.
This position reports to the Regional Primary Health Care Program Manager responsible for the service.
Flexible hours may be required (Combination of days, evenings, and weekends required).
Job Demands

Constantly moves, lifts, or operates equipment/machinery up to 10 lbs. and regularly is required to lift/move equipment, books, supplies, and sometimes children up to 25 lbs. Occasionally, may have to reposition a patient over 50 lbs.
Physical effort also includes regular sitting to conduct patient histories, performing work on the computer and when assessing patients, regularly stands or walks.
When performing procedures, regularly uses fine finger/precision work and hand tools (i.e. medical instruments, computer mouse, stethoscope, dopplers, etc.) requiring accurate control and steadiness.
Visual concentration is required when performing physical assessment and procedures, completing charts/reports, observing patients, reading equipment/monitors, and completing work-related correspondence on the computer.
Job Qualifications
Completion of an approved/recognized Nurse Practitioner program is required.
Current registration with the College of Registered Nurses of NL (CRNNL) and Licensed (or eligible for licensure) as a Nurse Practitioner (Family/All Ages) with the CRNNL is required.
A minimum of five years clinical experience within the last seven in Primary Health Care is required.
Current BCLS Certification is required.
Possessed strong abilities in the areas of collaboration, clinical reasoning, professional development, interpersonal effectiveness, conflict management and teamwork.
Demonstrated progressive clinical leadership skills.
Possessed strong communication, problem solving, decision-making, and teaching skills.
Demonstrated ability to function autonomously in an advanced practice role.
Support and participate in research to improve practice and client outcomes.
Demonstrated skills in computer and database management.
A valid Newfoundland and Labrador Class 5 Driver\'s license and use of private vehicle are conditions of employment.
A satisfactory record of work performance and attendance is required.
