Job Description

The Blossom Group provides homes for people with complex special needs. Blossom Group is a CARE based organization that provides therapeutic live in programs, we support children, youth, and adults through a range of cognitive, social, and emotional issues, including Autism Spectrum Disorders, neurobiological and developmental disabilities, and those with coexisting mental health and behavioural issues. We strive to improve health outcomes by supporting clients through personalized, accessible healthcare solutions. We are looking for a dedicated and compassionate Nurse Practitioner to join our team, working part-time 16-24 hours per week to serve our diverse community in Ontario.

Core Responsibilities



Provide comprehensive primary care services, including health assessments, diagnosis and treatment of acute and chronic conditions. Order and interpret diagnostic tests. Develop and implement patient care plans, with an emphasis on preventive care and health promotion. Work autonomously while being supported by our multidisciplinary team. Develop Trainings: for direct care employees on programs, plans and protocols using in-person and our online training platform Talent LMS. Collaborate with other healthcare professionals to provide holistic, patient-centre care. Provide patient and company wide education on health management and diseases prevention. Maintain accurate and timely documentation.

Qualifications and Requirements



Current registration with the College of Nurses of Ontario as a Nurse Practitioner. Registered Nurse in the Extended Class (RN EC designation). Current liability insurance. Master's degree in nursing with an NP designation. Minimum of 2 years of experience as an NP, ideally in primary care or community health. Excellent clinical skills with a strong commitment to quality care. Effective communication and interpersonal skills. Commitment to providing exceptional patient care.

Other Job Requirements



Must have own reliable transportation. Successful Criminal Record Check with VSC or BRC as per MCCSS requirements.
