About the Role:
The Nurse Practitioner (NP) works within an interdisciplinary team to provide primary health care with a strong emphasis on health promotion and disease prevention. The NP participates in the development, implementation, monitoring and evaluation of primary care services for individuals, families, and the community.

REPRESENTATIVE ACTIVITIES:
Provide ongoing comprehensive health care services in the areas of assessment, diagnoses, treatment, chronic disease prevention and management, health education and counselling
Work within an inter-professional team to provide quality evidenced based care
Keep complete, accurate and timely records of client visits using the electronic medical records system
Participate in case conferences and chart reviews
Assist with chronic disease and prevention initiatives, collaborate with family physicians, and advocate on behalf of patients.

QUALIFICATIONS:
3 years clinical experience
Baccalaureate or Master of Nursing degree with successful completion of a post graduate Nurse Practitioner program
Registration in the Extended Class and good standing with the College of Nurses
Current membership with NPAO, including professional liability insurance
Experience with youth, seniors and individuals with disabilities
Strong health assessment and clinical skills
Demonstrated commitment to an inter-professional team environment
Comfort with electronic medical record systems
OTHER
Ability to work both regular and extended hours in locations identified by RCHS
Valid Driver's license
Canadian criminal record check and a vulnerable sector check that are satisfactory to RCHS

Pay: $63.45-$74.65 per hour
