The South Georgian Bay Ontario Health Team

is seeking a Nurse Practitioner (NP) Locum to join a multi-disciplinary team to provide primary care services in a primary care clinic model. The clinic is in the community of Wasaga Beach in a modern, purpose-built setting. You will facilitate the continuity of primary health care by working collaboratively with our local physicians, and Interdisciplinary Health Professionals including, but not limited to, the Georgian Bay Family Health Team, South Georgian Bay Community Health Centre, Georgian Bay Family Health Organization. This is a Locum position covering a staff maternity leave.

Who is the South Georgian Bay Ontario Health Team?

The South Georgian Bay Ontario Health Team (SGB OHT) is a new model of organizing and delivering health care that better connects patients and providers in their communities to improve patient outcomes. We are comprised of eleven anchor partners and a strong network of supporting community organizations. SGB OHT members collaborate with patients, families, caregivers, and the community to co-design the best possible care for our region.

Key Responsibilities:

The

Nurse Practitioner Locum

uses research and evidence to inform clinical care, education, and planning.



As appropriate, the

Nurse Practitioner

Locum

leads or participates in quality improvement initiatives.



The

Nurse Practitioner Locum

builds strong collaborative relationships with key internal and external partners and stakeholders.



The

Nurse Practitioner Locum

conducts clinical assessments, diagnosis, intervention, escalation, and evaluation.



The

Nurse Practitioner Locum

prescribes pharmaceutical and non-pharmaceutical interventions.

What experience, education, and skills do you need?

Credentialing

Registration in good standing with the College of Nurses of Ontario (CNO).

Education

RN in the Extended Class with a Graduate degree.

Relevant Experience

Minimum 3 years' experience working in a community and/or primary care setting preferred.



Proven experience in covering practices or working as a locum.

Locum Requirements, skills, and key competencies include:

Valid Ontario driver's license and operational vehicle. Membership of professional associations and/or interest groups an asset. Ability to adapt to change, generate new knowledge and apply it in different ways to formulate and problem solve within a context of complexity and uncertainty is essential. Ability to support home visits in the South Georgian Bay region as required. Models and promotes core ethical practice and reflects an optimistic and positive attitude. Verbal and written communication, including effective use of computer programs like Microsoft Office, PS Suite experience an asset. Risk management and optimizing patient safety. Managing competing demands and priorities within defined timelines. Working autonomously and being self-directed.
