Objective:
Nurse Practitioners (NP's) are strategically positioned in primary care to play a vital role in supporting patients through education, disease management, and self care practices. In this role, you will provide comprehensive, holistic care to patients requiring palliative and end of life care, addressing physical, emotional, and psychosocial needs. You will build meaningful relationships with patients and families while delivering patient centered, compassionate care in collaboration with an interdisciplinary team.

Edmonton West Primary Care Network supports family physicians in improving primary care through strong relationships, collaboration, and innovation. Our interdisciplinary teams include Nurses, Dietitians, Pharmacists, Behavioral Health Consultants, Social Workers, and Exercise Specialists who work together to deliver comprehensive, patient centered primary care services to our community.
This position is delivered in partnership with Pilgrims Hospice Society, a charitable organization providing compassionate, family centered care to individuals and families living with life limiting illness, as well as support to those experiencing grief and loss. Through this partnership, care extends beyond the clinic and into patients' homes and hospice settings, ensuring coordinated, dignified, and responsive end of life care.
In this role, you can expect to:
Provide Direct Patient Care:
Plan and conduct regular home visits to patients receiving palliative and end of life care
Provide in person and virtual care to patients and families as required
May be required in certain scenarios to participate in an on-call schedule to ensure continuity of care, including overnight and weekend coverage
Assess, plan, implement, and evaluate care according to clinical guidelines and best practices
Manage symptoms and prescribe appropriate medications with follow-up
Engage patients and families in goals of care and advance care planning discussions
Advise patients and families on health promotion, disease management, and prevention
Perform patient advocacy and support informed decision making
Ensure accurate, objective, and timely clinical documentation
Collaboration and Team Based Care:
Work collaboratively with interdisciplinary team members, including physicians, nurses, home care, and community partners
Liaise with Pilgrims Hospice Society Home Visiting Hub physicians, nurses, and leadership to support coordinated care and 24-hour prescriber coverage participating in team rounds, handovers, and care planning meetings
Support referrals to appropriate healthcare professionals and services within the Edmonton zone
Contribute to continuity of care across home, hospice, and community settings
Provide medical care to clients enrolled in the Pilgrims Hospice Society Home Visiting Hub program
Support hospice coverage as needed and as scheduling permits
Provide guidance to patients and families regarding hospice services and transitions of care
Qualifications:
Master's degree in nursing is required
Five (5) years of professional nursing experience
Experience in community/continuing care/homecare is a strong asset
Case management and/or health system navigation is an asset
Proficiency in Microsoft Office Suite (including Word, Excel and Outlook)
Experience using EMR
License/Certification(s)/Other Requirements:
Current registration and good standing with the College of Registered Nurses of Alberta (CRNA) is required
Current Basic Life Support CPR (CPR-BLS) is required
Driver's license and vehicle is required
