Job Description


The Nurse Practitioner (NP) for the Hematology service of McMaster Children\xe2\x80\x99s Hospital will provide leadership and promote excellence in comprehensive care across the life spectrum of patients with inherited blood disorders - particularly Sickle Cell Disease and Thalassemia. Regarding providing comprehensive care, the NP will demonstrate excellent understanding of the inequities regularly encountered by populations experiencing inherited blood disorders \xe2\x80\x93 particularly Sickle Cell Disease \xe2\x80\x93 and will demonstrate experience working with these populations to mitigate these inequities and improve patient experiences and outcomes. The Nurse Practitioner is a core member of the interdisciplinary Hemoglobinopathy Clinic team. They will engage in family-centered and holistic nursing care through expert clinical practice, education, consultation and research. The NP will support the clinical activity of the Hematology service and the associated patient care and interventions with a focus on care for Hemoglobinopathy patients. The NP will collaborate with the interdisciplinary team and will be responsible to: provide direct patient care, coordination of care including transfusion therapy, discharge planning, education for patient, family and their communities, and the coordination of related community and other resources for this patient population. The NP will also be responsible to mentor nursing and other health professional students as appropriate, and participate in continuing education activities to integrate the role of the NP in the program and academic settings. This includes initiating, participating, and/or leading programs, presentations, publications, evaluations, research initiatives and other system changes to continue the enhancement of interdisciplinary care teams.

Qualifications

1. Current Certificate of Registration in the College of Nurses of Ontario as a Registered Nurse Extended Class RN(EC)
2. Completion of a Master\xe2\x80\x99s Degree in Nursing, with an Advanced Practice Nurse Certificate in Pediatrics, (NP Pediatrics) or Primary Health (NP PHC) preferred
3. Minimum of 3 years acute/primary clinical experience
4. Minimum of 3 years nursing experience preferred
5. Experience with chronic health maintenance including transitioning pediatric patients to adult care is an asset
6. Demonstrated experience providing equitable and inclusive care to parents, adults and children from historically excluded and marginalized communities including folks who are Indigenous, Black, racialized, disabled, and/or Two-Spirit and LGBTQIA+
7. Demonstrated understanding and commitment to equity, diversity and inclusion including evidence of education and learning undertaken in this space
8. Proficiency in performing history and examination of the infant, toddler, child, adolescent and early adulthood
9. Knowledge and assessment of developmental milestones at all stages of pediatric development and ability to access appropriate community or hospital-based resources in complex care setting
10. Experience and knowledge to recognize emergent hematology concerns and distinguish urgent from semi-urgent and non-urgent matters and condition
11. Ability to provide trauma-informed counsel and education to parents, adults and children in Hemoglobinopathy health related topics
12. Knowledge and experience engaging diverse community resources to best support patients with Hemoglobinopathy diagnoses
13. Able to handle significant daily psychological and emotional stress in dealing with parents of critically or chronically ill children and adults
14. Experience in the development and maintenance of medical directives
15. Demonstrated ability for independent action, initiative, problem solving, organization and prioritization
16. Demonstrated leadership, critical thinking, interpersonal and communication skills, both written and verbal
17. Demonstrated ability to work in a consultative role with the members of the multidisciplinary team
18. Demonstrated ability to be flexible and adapt positively to constantly changing clinical situations
19. Knowledge and experience in change theory and pediatric learning principles
20. Demonstrated ability to utilize information technology
21. Membership to professional organizations relevant to the role
