Job Opening: Nurse Practitioner (ER & Nursing Care Unit)
Position:
Part- time (8 -9 july )
Key Responsibilities:
Deliver comprehensive, client-centered care within your scope of practice.
Conduct assessments, interventions, monitoring, and follow-ups for acute and chronic conditions.
Perform client interviews and examinations, develop and implement treatment plans, integrating evidence-based health promotion strategies.
Adhere to the College of Nurses' standards of practice.
Actively engage in an inter-professional care model.
Participate in peer and chart audit reviews as needed.
Contribute to case conferences.
Work collaboratively within a team to support marginalized individuals with chronic illness, mental health, and addiction.
Participate in professional development opportunities and mentor Nurse Practitioner/medical students.
Maintain competencies through ongoing professional development.
Involve patients in all aspects of their care and treatment.
Educate patients on treatment administration, managing side effects, and follow-up planning.
Provide phone support when necessary.
Conduct community education sessions as scheduled.
Maintain professional relationships with other agencies, colleges, and universities.
Participate in the SCHC on-call schedule as per funder requirements.
Pursue continuing education opportunities as required.
Scope of Responsibility:
The Nurse Practitioner will practice within the full scope of their role, adhering to the College of Nurses of Ontario standards. This position reports to the Primary Care Manager.
Qualifications:
Master's degree as a Nurse Practitioner with a Primary Health Care certificate.
Active, good standing registration with the College of Nurses of Ontario in the Extended Class.
Member of the Nurse Practitioners' Association of Ontario (NPAO) with additional professional liability coverage.
3-5 years of experience as a Nurse Practitioner, utilizing advanced nursing assessment skills in an inter-professional care setting.
Pediatric experience is an asset.
Experience as a Nurse Practitioner in a community setting is preferred.
Experience in program development is a plus.
Current HCP CPR certification.
Willingness to join other professional organizations to enhance practice (e.g., Registered Nurses Association of Ontario).
Fluency in other languages relevant to the role is a plus.
Ability to work independently with strong self-direction, organizational skills, and confidence.
Excellent interpersonal, conflict resolution, and communication skills (both written and oral).
Working Conditions:
Flexibility in hours, with occasional evening or weekend work required.
Flexible work locations within the community.
Valid driver's license and personal vehicle required.
