With clinics in Victoria and Qualicum Beach and new expansions underway, we are building a team of providers who are passionate about making healthcare personal again.

The Opportunity

We are seeking a

Nurse Practitioner (NP)

with a strong interest and experience in

diabetes management and primary care

to join our collaborative, multidisciplinary team. This role is ideal for an NP who thrives in a supportive environment where they can truly make a difference in patients' lives while helping shape the future of community healthcare.

What You'll Do

Provide comprehensive

primary care services

, including assessment, diagnosis, treatment, and follow-up. Lead and manage care for patients living with

Type 1 and Type 2 diabetes

, including medication management, insulin initiation/titration, lifestyle support, and complication monitoring. Collaborate with physicians, pharmacists, dietitians, and allied health providers to ensure seamless, team-based care. Participate in developing and improving chronic disease programs within Care2Talk Health. Deliver both

in-person and virtual care

appointments, ensuring accessibility and continuity for patients. Contribute to the supportive, patient-first culture we are building across our network.
What We're Looking For

Current registration (in good standing) as a

Nurse Practitioner

with the BC College of Nurses and Midwives (BCCNM). Demonstrated experience or specialized training in

diabetes care and chronic disease management

. A strong commitment to

holistic, patient-centered care

. Comfort with electronic medical records (EMRs) and virtual care platforms (Accuro experience an asset). Excellent communication, teamwork, and leadership skills. Flexible and adaptable, with a passion for innovation in healthcare delivery.

Pay: From $95.00 per hour

Expected hours: No less than 8 per week

diabetes management: 2 years (required)
Licence/Certification:

licence with BCCNM (able to practice in BC) (required)
