Care2Talk Health is looking for a Nurse Practitioner (NP) to support our Edmonton site and deliver telehealth services. *2 days a week in-person care, 3 days a week virtual care.

The Role:

The Nurse Practitioner (NP) will play a central role in

chronic disease management with a primary focus on diabetes care

. This position involves the comprehensive assessment, diagnosis, and ongoing management of patients living with diabetes and related conditions. The NP will develop individualized treatment plans, prescribe medications, monitor outcomes, and coordinate care across interdisciplinary teams.

Care will be provided through a

hybrid model

of in-clinic and telehealth visits, ensuring patients have access to timely and personalized support. The NP will also educate patients and families on disease progression, medication adherence, lifestyle changes, and self-management strategies, translating complex health information into clear, actionable steps. Strong interpersonal skills and a compassionate approach are essential to supporting patients through long-term care journeys.

Key Responsibilities:

Conduct patient assessments, focusing on diabetes and other chronic conditions (e.g., hypertension, cardiovascular disease). Develop, implement, and adjust

personalized care plans

, including prescribing medications, ordering diagnostic tests, and initiating referrals. Provide ongoing

patient education and counseling

to support disease self-management, lifestyle modifications, and treatment adherence. Monitor and track

key diabetes indicators

(A1C, blood glucose levels, kidney function, etc.) and respond with timely interventions. Collaborate with physicians, pharmacists, dietitians, and other healthcare professionals to coordinate care for complex and specialized cases. Deliver care across

both in-person and telehealth settings

to improve accessibility and continuity. Support patients with co-morbidities and senior populations in navigating community health resources and specialized services.
Qualifications:

Primary care experience preferred

, ideally within chronic disease management. Demonstrated expertise in

diabetes management

, including patient assessment, individualized treatment planning, and long-term follow-up. Knowledge of insulin therapy, oral hypoglycemic agents, and other diabetes-specific pharmacological and non-pharmacological interventions. Experience in

patient education and empowerment

, including motivational interviewing and behavior-change counseling. Strong understanding of community-based health services and referral pathways for chronic care. Previous experience supporting

senior populations

and patients with multiple co-morbidities is an asset. Proven ability to work collaboratively in an

interdisciplinary care team

Pay: $230,000.00-$250,000.00 per year
