The Bilingual Neurology Nurse Field Case Manager (FCM) is responsible for a collaborative process of ongoing assessment, planning, facilitation, service coordination, evaluation, and advocacy to meet the patient /physician/client needs through flawless customer service. The Nurse Field Case Manager will deliver through expert communication skills and available resources, quality and cost-effective treatment outcomes for the patient. The Nurse Field Case Manager will work directly with the reimbursement team to help secure drug coverage.Nature and Scope
The Nurse Filed Case Manager will help support a part-time caseload in the province of work that includes disease status, education and trainings provided to patients and providers, coordination of services, in-clinic or telephone support, adherence monitoring and side effect management. Building strong working relationships, collaboration, effective communication, and attention to detail both internally and externally are key responsibilities to meet specific team and business objectives. In addition, the Nurse Field Case Manager will attend pre-booked training sessions, complete self-learning assignments and independent studies while adhering to SDM Specialty Health Network Standard Operating Procedures, Policies, and the College of Nurses Standards of Practice.What you will do:\xc2\xb7 Responsible for a patient and physician/clinic case load\xc2\xb7 Work from home with up to approximately 50-75% of travel within provided territory, this will include occasional overnight stays.\xc2\xb7 Act as the primary resource & contact for patients, physicians, Health Care Professionals, and vendors.\xc2\xb7 Coordinates & facilitates the client progress from enrollment and throughout therapy\xc2\xb7 Ensure thorough documentation in patient file to ensure continuity of care is maintained and that any changes in patient care are clearly identified and transitioned accordingly\xc2\xb7 Provide status updates related to reimbursement; patient training requirements; pharmacy dispensing/ delivery/refill requests\xc2\xb7 Report all Adverse Event (AE) and Product Technical Complaints (PTC) immediately or as soon as possible upon becoming aware of the event as per current policies and procedures.\xc2\xb7 Promotes communication and coordination between the MDs/pharma/patients/clinic staff and other departments within SDM Specialty Health Network\xc2\xb7 Provide in-clinic, in-home, Long-term care, Hospital training & build relationships on behalf of the program\xc2\xb7 Provided face to face and virtual education to patients, care partners and healthcare team\xc2\xb7 Work collaboratively with the Pharma company representatives\xc2\xb7 Respect confidentiality, advocate for patients/families to facilitate positive outcomes, and ensure that ethical issues are identified and resolved according to the Canadian Nurses Association Code of Ethics\xc2\xb7 Demonstrate commitment to Continuous Quality Improvement\xc2\xb7 Maintain high standards of professional practice by adhering to the ethics of the profession and keeping abreast of the latest professional advances\xc2\xb7 Assume responsibility for personal and professional development\xc2\xb7 Other responsibilities as required by SDM Specialty Health NetworkWhat you will need:\xc2\xb7 Registered Nurse with a current, unrestricted license in the province of employment. BScN preferred, however, a combination of experience and education will be considered\xc2\xb7 Disease state experience in Neurology (Parkinson\xe2\x80\x99s disease) is an asset\xc2\xb7 Proficient in reimbursement landscape\xc2\xb7 Recent experience with Patient Support Programs (PSP)\xc2\xb7 Demonstrated customer/account focus and consistently deliver on the PSP value proposition\xc2\xb7 Fluently bilingual in both written and spoken languages (French & English)\xc2\xb7 Five or more years direct clinical experience\xc2\xb7 Strong telephony skills\xc2\xb7 Demonstrated leadership experience and ability to work under pressure\xc2\xb7 Ability to provide an organized, efficient approach to case management\xc2\xb7 Possession of excellent skills in assessment, communication, critical thinking and decision making\xc2\xb7 Ability to demonstrate understanding of SDM Specialty Health Network policies, legal considerations and professional ethics\xc2\xb7 Proficiency of computer skills including; Microsoft applications (Outlook, Word, Excel and Internet Explorer); experience with web based applications (Salesforce)\xc2\xb7 Ability to adapt to a changing environment\xc2\xb7 Commitment to the principle of quality client-centered service delivery\xc2\xb7 Knowledge of case management processes\xc2\xb7 Valid driver\xe2\x80\x99s license and with reliable vehicleWhy work at Shoppers Drug Mart?Acting as consultants, supporting internal customers and the Associate-Owners, you will get experience unique to the retail industry while working for an award-winning national brand.
