POSITION SUMMARY
Reporting to the Samaritan Place Spiritual Care & Mission Effectiveness Director, the Music Therapist is a member of the Interdisciplinary team and works in close partnership with the Holistic Care Team in utilizing professional methodology and practice to provide support to patients and their defined family.
The role of the Music Therapist is to contribute to the holistic care of patients and families, be it physical, spiritual, or psychological in nature through the use of music. The Music Therapist honors all persons and is respectful of their unique journey and the interweaving of health, cultural, philosophical and/or spiritual tradition.
This includes collaborating with the Holistic Care team to provide intervention that promotes or restores the patient's capacity for optimal psychosocial well-being; providing leadership in developing, implementing and evaluating music therapy interventions and programs that are meaningful and valued by patients, families, and the Interdisciplinary team; and representing Hospice at Glengarda in the community.
PERFORMANCE REQUIREMENTS
Clinical
Provides assessments from a Music Therapist perspective, and maintains professional ethics and standards of practice. The Holistic Care assessment tool and response will be a collaborative team approach. As part of the Interdisciplinary team, Music Therapist documents information on the patient chart that informs care; communicates regularly with other members of the team, and attends rounds
Provides individual support for patients and families making the adjustment to the hospice environment; identifying meaning, connection, values, areas of distress, and assisting patients to gain a sense of home, both physically and emotionally
Coordinates referrals and services referrals from other members of the Interdisciplinary team and provides follow-up as appropriate
Assists Spiritual Care Practitioner in the coordination of special music programing, as required
Assists Spiritual Care Practitioner with Celebration of Life services as required
Oversees the care and maintenance of musical instruments
Organizational
Promotes the Vision, Mission, and Values of Emmanuel Health in Hospice at Glengarda, in interactions with patients, families, other members of the Interdisciplinary team, volunteers and the community
Supports the organization, staff and patients in ethical reflection and practice
Works closely with Music Therapist at St. Paul's Hospital to build comprehensive and sustainable programs
Promotes a culturally sensitive hospice environment that embraces and respects the diversity of our patients and their families
Evaluates the Music Therapy programming on an on-going basis
Develops metrics regarding Music Therapy activities and effectiveness
Participates in and assists Spiritual Care Practitioner preceptorship to CPE students as required
* Co-ordinates opportunities for supervision of Palliative Care Music Therapy internship
