The goal is to share information about community assets/resources, foster effective teamwork across communities/departments/services/programs with a view to improving clients access to community- and team-based services. The PHCAC will also establish mechanisms to involve relevant care providers, clients/families, and other stakeholders in the planning for development and direction of future PHC programming. The PHCAC will participate/lead relevant community meetings and relevant councils, roundtables, and committees.

Monitoring communication processes, workload, and reporting outcomes: The PHCAC will be responsible for developing appropriate practice processes and agreements required to capture the clinical workload and documentation of providers.
