Scope and Summary of Primary ResponsibilitiesThe Client Care Navigator (CCN) coordinates access to vision rehabilitation services for Indigenous (and non-Indigenous) clients with vision loss.Through community outreach, the CCN ensures Mikmaw people with vision loss in Nova Scotia have access to culturally safe and client-centered care informed by Indigenous perspectives, expertise and Indigenous led collaboration and delivered in a manner that respects their cultures and contributions.II. The role of a Client Care Navigator includes:

• Building relationships and partnerships with Mi'kmaw communities.
• Completing intake assessments with clients
• Scheduling VLRC appointments with service providers
• Monitoring and providing proactive follow-up for clients ensuring ongoing collaboration across services/providers.
• Ensuring all service-related activities provided are documented in the client health record following VLRC policies.

RequirementsIII. QualificationsKnowledge and Skill Requirements

• Demonstrated knowledge, understanding and application of cultural safety and competency within healthcare would be considered an asset.
• Strong understanding of Mikmaw, culture, customs, and traditions.
• Knowledge of spoken and written Mikmaw language considered an asset.
• Knowledgeable about services available in Mi'kmaw communities.
• Knowledge of client-centered philosophy related to interviewing, assessments, and scheduling practices.
• Excellent communication and interpersonal skills, including shared decision-making and facilitation.
• Able to prioritize workload and manage competing tasks with excellent problem-solving skills.
• Proficient in technology (e.g.: Microsoft office, case management and care coordination systems)

Experience and Education

• Registered health professional designation is an asset.
• Minimum bachelor's degree in Social Work, Gerontology or other related health care discipline.
