Position Title: Mi\xe2\x80\x99kmaw Client Care Navigator

Location: Halifax, Nova Scotia

Reports To: Provincial Manager, Healthcare Operations

Term: Full-Time, permanent

Date: 1st December 2023

I. Scope and Summary of Primary Responsibilities

The Client Care Navigator (CCN) will act as a resource to coordinate and integrate supports and services for both Indigenous clients and non-Indigenous clients with vision loss to access culturally safe and client centered care. The CCN provides support and advocacy for Indigenous clients and their families by facilitating and coordinating access to vision rehabilitation services, and networking with the Nova Scotia Mi\xe2\x80\x99kmaw health systems and community partners that provide services to Mi\xe2\x80\x99kmaw people.

II. The role of a Client Care Navigator includes:

  • Facilitating relationship building between Mi\'kmaw community health programs and other community partners
  • Completing intake assessments with clients
  • Scheduling all VLRC appointments with service providers
  • Monitoring and providing proactive follow-up for clients ensuring ongoing collaboration across services/providers.
  • Ensuring all service-related activities provided are documented in the client health record following VLRC policies.

III. Qualifications

Knowledge and Skill Requirements
  • Demonstrated knowledge, understanding and application of cultural safety and competency within healthcare would be considered an asset.
  • Strong understanding of the Mi\xe2\x80\x99kmaw language (including spoken and written), culture, customs, and traditions
  • Knowledgeable about services available in Mi\'kmaw communities.
  • Strong knowledge of client-centered philosophy related to interviewing, assessments, and scheduling practices.
  • Excellent communication and interpersonal skills, including shared decision-making and facilitation.
  • Able to prioritize workload and manage competing tasks with excellent problem-solving skills.
  • Proficient in technology (e.g.: Microsoft office, case management and care coordination systems)
Experience and Education
  • Registered health professional designation is an asset.
  • Minimum bachelor\'s degree in Social Work, Gerontology or other related health care discipline.
