Status: Full-time, Permanent
Schedule: Monday - Friday, Standard 38.75 hour work week
Evening and weekend hours may be required
Location: Primary Care Network Wellness Centre at #4000, 1800 194 Avenue SE; Calgary, AB
Eligible for hybrid remote work
Reports to: Program Manager, Mental Health & Community Wellness
Position Summary

This position encompasses a wide range of patient focused duties which include working as part of an interprofessional team serving community-based patients within the South Calgary Primary Care Network. In this position, the Therapist is responsible for delivering a range of services including; psychosocial assessment, consultation, and counselling services in accordance with evidence-based treatment models and professional standards of practice. The position involves direct and indirect collaboration and coordination of patient centered care with internal and external providers as well as liaising with relevant communities and external services.

What Does the Role Do?

Therapy


  • Provide short-term, strength-based counselling services to individuals, couples and families experiencing psychological or emotional challenges.

  • Create and maintain a therapeutic environment that is client-centered, goal-oriented and driven by client-led therapeutic goals.

  • Ensure clinical theory supports evidence-informed treatment recommendations.

  • Actively elicit feedback from the patient regarding the quality of the therapeutic relationship in the interest of enhancing therapeutic alliance.
  • Coordinate referrals to community mental health programs and services, as appropriate.
Group Facilitation
  • Facilitates group workshops of a variety of mental health topics to patients of all ages, learning styles, and skill levels.
  • Negotiates/facilitates conflicts or disagreements that may arise during the group workshops.
  • Works collaboratively with members of the interdisciplinary team members of other departments to develop, facilitate, and evaluate group workshops.
  • Provides educational sessions to physicians, community members, and other health team members, as required.
General duties

  • Support a collaborative working relationship with family physicians and the interprofessional team, through regular case consultation, care coordination, and shared implementation of patient-centered interventions

  • Participate in relevant professional development activities and regular individual supervision sessions with Clinical Supervisor.

  • Collaborates with the Program Manager and team and to identify and develop unique approaches to address patient needs and maximize patient care outcomes.

  • Supports the orientation of new members and students under the direction of the Clinical Supervisor.

  • Actively participate in meetings, ongoing program development, evaluation and quality improvement opportunities.

  • Acts as a resource for other staff, as appropriate.

Who Should Apply?
  • Master\xe2\x80\x99s degree in Psychology, Counselling, or Social Work
  • Registration with either the College of Alberta Psychologists or Alberta College of Social Workers (With Psychosocial Interventions Authorization).
  • Preferred minimum of 5 years\xe2\x80\x99 experience providing individual, family, and/or group therapy to adults, adolescents, and children.
  • Experience facilitating individual, couple, and group sessions
  • Experience in primary care and/or knowledge of primary health care reform and medical home implementation preferred.

  • Proficient with computers and technology; EMR experience preferred

Working Conditions

  • Normal office environment
  • Evening and weekend hours may be required
  • Valid driver\xe2\x80\x99s license with access to a reliable vehicle as minimal travel is required
  • Two supervisory references required
  • Criminal and vulnerable sector background check required
  • Proof of COVID vaccination required
