About the Role



The Triage and Transition team works with families beginning their journey at CASA Mental Health, and those who are ready to transition out of CASA programs into the community. The team eliminates gaps in services, while offering a streamlined intake experience and a warm hand-off to community partners. Triage and Transition improves the patient's journey through CASA as well as the service provider's experience at CASA by way of ensuring the right patient is accessing the right program.



The Mental Health Nurse will:

Conduct comprehensive bio-psycho-social mental health assessments and formulate clinical diagnoses using DSM-TR and other diagnostic tools. Have a strong understanding of psychosocial and neurobiological development in children, youth and young adults and psychotropic medications. Incorporate objective findings and collection of collateral info, diagnostic findings, specialist reports and history into assessments. Provide therapeutic interventions and crisis intervention/management, risk assessment and safety planning. Knowledge and utilization of variety of developmentally appropriate treatment approaches including dialectic behaviour therapy, solution focus, motivational interview and resiliency building modalities. Conduct case conferences and liaise with CASA therapists and psychiatrists, community services and other professionals involved with the child and their family. Coordinate consultation with other therapists or external partners as appropriate. Integrate assessments and other data to establish appropriate clinical pathways and provide nursing interventions to promote, maintain and restore health.


Qualifications

Bachelor's degree in nursing or psychiatric nursing. A minimum of three to five years of recent pediatric experience in an ambulatory care setting which has included the care and management of children with mental health problems. Acute care experience in children's mental health would be an asset. Excellent verbal and written communication skills, including use of an electronic medical record system. Must be a member in good standing with the College of Registered Nurses of Alberta or College of Registered Psychiatric Nurses of Alberta.


Conditions of Employment

A satisfactory Criminal Records Check with Vulnerable Sector Search and Child Intervention Check. This would be the financial responsibility of the successful candidate. Must maintain Therapeutic Crisis Intervention (TCI) certification (training provided by CASA) and be physically able to perform all interventions.
