Reporting to the Manager of Specialized Services, the incumbent will be responsible for the following functions: \xc2\xb7 Conducting and completing the detailed Community Mental Health Assessment and SBAR notes for individuals referred; \xc2\xb7 Assessing, screening, referring and linking individuals to internal CMHA services or alternate community resources; \xc2\xb7 Compiling and gathering collateral information; \xc2\xb7 Participating in Case Disposition meetings as needed to determine outcomes for individuals referred to the Agency; \xc2\xb7 Consulting with the Mental Health First Response Team to prioritize individuals awaiting service; \xc2\xb7 Communicating and coordinating relevant information with Agency staff; \xc2\xb7 Attend as needed weekly psychiatry rounds; \xc2\xb7 Providing intervention and brief solution focused therapy support to individuals experiencing a psychiatric crisis (intervention may be via telephone or by personal contact); \xc2\xb7 Providing a quick response to urgent situations that arise; \xc2\xb7 Linking individuals with urgent needs to immediate resources (ie securing food resources, financial assistance, and housing). Qualifications: Education required: \xc2\xb7 Post-secondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W; a Registered Nurse or Registered Practical Nurse with current registration with the C.NO. (Preference to those with Canadian Certification in Psychiatric/Mental Health Nursing); Bachelor\xe2\x80\x99s Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W; or a post-secondary degree with a relevant professional registration and/or Grandfathered Post-Secondary Degree. \xc2\xb7 Experience and a detailed knowledge of serious mental illness, usually gained by several years of working with this population. \xc2\xb7 Valid Ontario Driver\xe2\x80\x99s Licence and access to a vehicle are required. \xc2\xb7 Preference will be given to a qualified candidate who is proficient in both official languages. Hours of Work: 35 hours/week 9:00am-5:30pm. Flexible hours are required to meet the position requirements Salary: Commensurate with experience within the range: $52,864 - $75,242, inclusive of HOOPP.
