The WestView Primary Care Network (WVPCN) is a dynamic team of healthcare professionals working alongside family physicians and nurse practitioners to support patients in achieving their best possible health. Serving the communities of Spruce Grove, Stony Plain, Parkland County, and surrounding areas, the WVPCN offers a range of programs that address physical, mental, and social well-being. Our collaborative, community-based approach ensures patients receive the right care, in the right place, at the right time.

We are currently recruiting two full-time Mental Health Community Navigators (2.0 FTE) to join the PCN team in an innovative position designed to help patients access timely mental-health supports and navigate Alberta's mental-health system with ease and compassion.

Position Summary


The Mental Health Community Navigator (MH-CN) is part of the WestView Primary Care Network's Social and Community Health Program and works in collaboration with member clinics to ensure patients can access same-day or next-day (or close to) supports to navigate Alberta's mental-health system. The MH-CN provides timely, compassionate, and practical assistance to help patients connect with appropriate community and healthcare services, addressing barriers that impact mental-health and social well-being.

Key Responsibilities



Respond to referrals from member clinics to support patients in accessing mental-health and community resources quickly and effectively. Provide same-day or next-day (or close to) navigation and connection to appropriate mental-health, social, and community supports. Collaborate with primary-care teams to identify patient needs and assist in developing individualized support plans. Assist patients in understanding and navigating Alberta's mental-health system, including referral pathways, intake processes, and available services. Help individuals address social determinants of health that may affect their mental health, such as housing, financial insecurity, or social isolation. Encourage communication and coordination among patients, primary-care providers, and external agencies. Provide short-term, solution-focused support to help patients engage with longer-term mental-health or community services. Maintain accurate and timely documentation in the electronic medical record, adhering to privacy and confidentiality standards. Participate in case discussions, team meetings, and community partnerships to enhance coordination and promote mental-health awareness.

Qualifications



Post-secondary education in social work, psychology, human services, or a related field (diploma or degree preferred). Demonstrated experience supporting individuals with mental-health needs or navigating community resources. Strong understanding of mental-health and social-service systems within Alberta. Excellent communication, problem-solving, and organizational skills. Demonstrated ability to work collaboratively in a multi-disciplinary environment. Cultural awareness and sensitivity in supporting diverse populations. A valid driver's license and access to a reliable vehicle. A current Criminal Record/Police Information Check with Vulnerable Sector clearance. Training in Mental Health First Aid, ASIST, or NVCI is an asset. Fluency in multiple languages is an asset.

Reporting Structure


This position reports directly to the WVPCN Social Worker and works closely with member-clinic teams.
