Position Summary:



The Mental Health Systems Navigator primary role is to use their specialized knowledge of mental health services and supports to create effective links between appropriate mental health and addictions services, and social and community resources. The Navigator/Advisor is a specialized resource who assists in supporting MBQ's client-facing staff in the care planning for their adult, youth and child clients by connecting clients to the most appropriate service provider and liaising with other parts of the health care and community support system within the province.

The position supports MBQ's ability to connect clients to appropriate Mental Health and Addictions services and supports that are client/family centered, wholistic in nature, focused on prevention, promotion and early intervention, and firmly rooted in Mohawk traditions and cultural values.

Reporting:



This position will report to the Director of Community Wellbeing.

Summary of Main Responsibilities:



Collaboration, Partnerships and knowledge exchange



o Be available to MBQ staff to consult and support regarding Mental Health and Addictions resources available for their client(s)

o Provide MBQ staff with information to help them navigate the Mental Health and Addictions system on behalf of clients using existing databases and available tools of communication.

o Assist with the development and documentation of available province-wide Mental Health and Addictions supports and services particularly for indigenous, first nations and metis peoples.

o Actively and independently keep up-to-date on mental health and addictions services and community supports available throughout Ontario

o Manage the intake and track progress of clients through MH&A services

o Liaise with other MBQ mental health services and supports on cases

o Develop a communication strategy, including education and awareness-building materials, required to support Navigator/Advisor services, and to encourage client-facing staff to identify and also self-identification of Indigenous, First Nations and Metis who would benefit from services and supports

o Build awareness about the Navigator/Advisor role internally with MBQ client-service staff by actively networking and connecting to support existing clients

o Build awareness about the Navigator/Advisor role to other service providers and relevant community partners

o Develop strategic collaborative partnerships within the community and with other partner organizations throughout Ontario

o Conduct outreach via attending meetings, hosting presentations, hosting gatherings, and public information sessions and events

Quality Improvement



o Develop tools for collection of data as part of service evaluation plan

o Monitor, assess and evaluate the uptake of the program on an ongoing basis

o Prepare and submit reports to the Mental Health and Addictions Navigator evaluation framework.

o Utilize results to improve/expand role and its services

Professional Development



o Participates in training activities as appropriate to improve skills and knowledge base.

o Keeps current on organizational policies and procedures, relevant legislation and their updates.

o Completes training to meet organizational needs as identified for the position and as an employee of Mohawks of the Bay of Quinte

Health and Safety:



o Promoting and following all MBQ Health and Safety policies and procedures

All other duties as required



Requirements:



Degree in Nursing (BScN) or Diploma of Nursing (required) or Social Work Degree (BSW/MSW); Valid certificate of registration with the College of Nurses of Ontario, Ontario College of Social Workers (required); Equivalent of 5 or more years of working experience in related capacity with similar responsibilities; Minimum two (2) years' experience working in Community Health (asset). Experience in a First Nations setting is an asset Combination of training, education and experience may be considered (at the discretion of the organization) Must provide a satisfactory Criminal Record Check (documentation will be required if successful applicant) First Aid and CPR an asset (and must be willing to take the training if hired) Valid Driver's License and vehicle
