Mental Health & Addictions Nurse (MHAN)

REPORTS TO: Patient Services Manager

LOCATION: Home and Community Care Support Services Waterloo Wellington

OFFICE BASE: Cambridge

STATUS: Permanent, Part-Time

SCHEDULE: 7 hours/day, prescheduled Monday-Sunday

UNION: ONA

ASSIGNMENT: 0.4 FTE

KEY RESPONSIBILITIES

Reporting to the Patient Services Manager, the incumbent will be responsible for:

  • Ensuring effective support for the Mental Health population to ensure communication and linkage with longer-term community support, primary care and providing timely and effective Mental Health nursing in the home
  • Providing the first in-home nursing visit within five (5) days from hospital discharge or community referral. During this visit, the nurse will confirm the patient hospital discharge care plan and perform medication reconciliation for the client.
  • Assessing mental health status
  • Providing ongoing risk assessment
  • Monitoring medication management
  • Liaising with attending physicians, psychiatrists and other support services
  • Providing education and supportive counseling
  • Assessing physical health and providing clinical nursing care where appropriate
  • Providing education regarding mental health, medication and coping strategies to patients, family and caregivers
  • Clinical interventions such as basic wound care, injections and other hands on nursing skills may be required
  • Performing other duties as assigned.
EDUCATION AND EXPERIENCE
  • 3 \xe2\x80\x93 5 years of relevant recent experience as a Registered Nurse in Mental Health
  • CNA Mental Health Certificate, or a comprehensive Mental Health Education Program that has been reviewed and approved by Home and Community Care Support Services Waterloo Wellington in writing
  • Registered Nurse (BScN or diploma)
  • Certification in mental health nursing
  • Proficient in the operation of a personal computer in a networked environment using a variety of computer software and data entry skills, i.e. Excel, Data Base Software, as well as various other programs
  • Knowledgeable in the use of technology in a community setting \xe2\x80\x93 e.g., lap top in the home, iPhone, remote networking, etc.
  • Demonstrated skill in setting and meeting deadlines
  • Demonstrated effective use of time and tools to manage work efficiently
  • Demonstrated experience in comprehensive assessment, use of different or combined approaches
  • Demonstrated skills in synthesizing information in order to plan and adjust services and to assist clients with informed decision-making
  • Demonstrated skill in crisis management and risk situations
  • Requires analysis and critical thinking skills
  • Working knowledge of community resources and roles of health care professionals
  • Solid knowledge of health care related legislation and practices
  • Knowledge of direct care/case management models used in community health care organizations
  • Knowledge of HCCSS priorities, policies, practices and service standards
  • Effective interpersonal and communication skills
  • Effective organizational and planning skills
  • Basic proficiency with computerized information systems
  • Effectively maintain a constant flow of verbal and written communication with others throughout the workplace as well as outside the organization
  • Able to communicate with clients\xe2\x80\x99, their families, and other relevant individuals in order to follow through with care plan directives
  • Demonstrated awareness of cultural diversity, as well as ability to behave discreetly and sensitively to confidential issues
  • Fluency in French language preferred
This position may be required to provide relief at other Home and Community Care Support Services locations or teams as needed and may be part of the on call rotation as per the provisions of the ONA collective agreement.
